L'examen gynécologique est généralement décrit comme désagréable et donc peu apprécié des femmes. La HAS et le CNGOF le recommandent systématiquement tous les ans, sans en préciser les objectifs.Nous avons réalisé une enquête par entretiens auprès de 14 médecins généralistes du Calvados et de 7 femmes, afin d'évaluer le vécu de cet examen par les praticiens et les patientes, et l'application des recommandations actuelles en pratique pour ainsi en identifier les freins. Notre travail a mis en évidence une théorie parfois difficile à exécuter : possibles difficultés techniques même pour des praticiens expérimentés, remise en question fréquente de leurs compétences concernant la sensibilité de leurs toucher vaginal et palpation mammaire, refus de certaines patientes principalement jeunes ou vivant en milieu rural, manque de courage et de disponibilité du médecin parfois, conviction de certains praticiens de l'absence de nécessité d'un examen annuel ou méconnaissance de cette recommandation. Le suivi gynécologique des femmes est donc très hétérogène. Seule la notion de confiance dans la relation médecin-patiente fait l'unanimité, favorisant l'acceptation de cet examen. Afin d'homogénéiser le suivi des femmes, il serait intéressant de réaliser d'autres études qui permettraient d'argumenter les recommandations françaises : elles pourraient porter sur la fiabilité et la sensibilité de l'examen gynécologique pour le dépistage des principales pathologies gynécologiques. En effet, à l'heure actuelle, ni les données de la littérature internationale, ni notre enquête ne permettent de conclure sur la périodicité idéale de l'examen gynécologique systématique.
