La morphologie et les dimensions de l'espace biologique ont été bien décrites, tant pour les dents que pour les implants dentaires. Des similitudes tant dimensionnelles que physiologiques ont été mises en évidence. Les variations dans la position et le contour des tissus péri-implantaires vont dépendre de la technique de pose chirurgicale et du design de l'implant. Le remodelage de la crête osseuse est dû à la réorganisation des tissus mous afin d'établir un espace biologique approprié autour des implants. La résorption osseuse se fait jusqu'à environ 2 mm du microgap en direction apicale. La première partie traite de la morphogénèse de l'espace biologique péri-implantaire: néoformation d'une attache épithéliale et d'une attache conjonctive, tout en évoquant les caractéristiques cliniques des tissus mous péri-implantaires, puis dans un deuxième temps, nous observons les effets des différents protocoles chirurgicaux, l'état de surface, le concept de "platform-switching" avec la situation et la forme de l'épaulement, le type de connexion prothétique ou le moment de la mise en situation ou de la mise en charge constituent autant de facteurs susceptibles d'influer sur les tissus péri-implantaires. L'objectif final étant de déterminer un type d'implant idéal en termes de design et de protocole chirurgical qui permette l'obtention d'un résultat esthétique optimum quant à la gestion des tissus mous et durs.

Le mémoire traite de la dynamique tissulaire après la mise en place chirurgicale d'un implant dentaire dans l'os. Les tissus muqueux ainsi que le tissu osseux péri-implantaires se modifient de façon à rétablir un espace biologique physiologique compatible avec l'implant, réalisant ainsi la parointégration de celui-ci. Les moyens de limiter ces transformations tissulaires d'adaptation, afin d'en contrôler les conséquences négatives comme la cratérisation osseuse crestale, et de permettre des résultats prévisibles et reproductibles sont proposés dans ce mémoire. Nous étudierons en particulier le choix du dispositif transmuqueux (pilier implantaire) et son incidence sur le remodelage osseux crestal, ainsi que le positionnement tridimensionnel de l'implant et son incidence sur le profil d'émergence. Nous conclurons ce travail en disant que connaître et préserver l'espace biologique participe à l'intégration de l'implant dans la cavité buccale.
