Introduction : Du fait de l'intrication des pathologies médicales, des problématiques sociales et de l'atteinte des fonctions cognitives, la personne âgée est à risque d’être hospitalisée pour difficultés de maintien à domicile lorsque les ressources locales (humaines, matérielles) deviennent insuffisantes. Objectifs : Repérer en situation de soins primaires les personnes à risque de maintien à domicile difficile permettrait d’anticiper leurs besoins et d’instaurer les aides nécessaires par le biais de la filière gériatrique afin d’éviter certaines hospitalisations. Méthode : Etude descriptive rétrospective de mai à octobre 2011 à partir des données médico-sociales des personnes de plus de 75 ans adressées dans un service de gériatrie aiguë pour maintien à domicile difficile. Comparaison des caractéristiques des patients pris en charge par la filière gériatrique avec les autres. Résultats : L’âge moyen des 61 patients inclus était de 85,9 ans. Les pathologies les plus fréquentes étaient les pathologies cardio-vasculaires (75%) et la démence (85%). Ils vivaient seuls dans 62,3% des cas. Le groupe non suivi par la filière présentait une majorité de patients dénutris (59,2% vs 32,3%, p=0,03) et moins de troubles du comportement (29,6% vs 55,9%, p=0,04). Les patients suivis par la filière étaient plus souvent en situation de réel maintien à domicile difficile que les autres à leur arrivée en unité de gériatrie aiguë. Conclusion : Le repérage par le médecin traitant des patients à risque de maintien à domicile difficile ainsi que la collaboration précoce avec la filière gériatrique locale permettrait d’éviter un certain nombre d’hospitalisations.

Objectifs : étude rétrospective ayant pour but de rechercher l'impact du motif « maintien à domicile impossible » chez les patients hospitalisés en gériatrie aiguë sur le retour à domicile à 2 mois. Résultats : en analyse univariée les patients ne rentrant pas à domicile ont plus souvent un critère d'hospitalisation MADI , ils ont plus d'atteintes neurologiques, de troubles cognitifs, de troubles du comportement, de diabète, de cancer actif ou en suivi. Ils sont plus dépendants, avec un score ADL plus faible et avec plus de troubles sphinctériens. Ils ont plus de passages infirmiers et d'auxiliaires de vie à domicile. Ils ont un IMC plus faible et une albuminémie moins élevée. En analyse multivariée, trois facteurs indépendants sont associés à un non-retour à domicile à 2 mois : une baisse de l'indice de masse corporel (IMC), une baisse du score ADL, et la présence de troubles cognitifs. Conclusion : le motif d'hospitalisation MADI n'est pas un critère indépendant de non-retour à domicile à 2 mois lors d'une hospitalisation en gériatrie aiguë.
