De part leur fréquence, leur potentielle gravité et les coûts sociaux et médicaux qu’elles engendrent, les chutes représentent chez le sujet âgé un important problème de santé publique. Les causes de chute sont polyfactorielles. Nombreuses sont les classes pharmacothérapeutiques susceptibles d’entraîner une chute. Ce travail détaille le rôle respectif des psychotropes et des traitements à visée cardiovasculaire. Les premiers, quels qu’ils soient, sont associés à un risque accru de chute, les seconds, seulement en cas de prescriptions inadaptées ou de surveillance insuffisante. L’étude réalisée : « Influence de la consommation de psychotropes sur la fréquence des chutes des patients âgés en institution » ne corrobore pas ces résultats. En effet, le pourcentage des patients ayant chuté chez les consommateurs de psychotropes est inférieur à celui observé chez les non consommateurs. Les facteurs de risque de chute chez les sujets âgés sont multiples et seule une analyse multivariée sur une importante population pourrait montrer la prédominance du facteur de risque « prise de psychotropes ». Après une chute, il convient d’en apprécier rapidement la gravité, d’en rechercher les déterminants, qu’ils soient particuliers au patient ou à son environnement, afin d’en réaliser l’éviction. En complément de l’enquête étiologique, la prévention secondaire des chutes comprend une prise en charge psychologique, une rééducation adaptée et l’aménagement de l’environnement matériel et humain. La médication, lorsqu’elle est identifiée comme facteur favorisant, est un élément que l’on peut contrôler.Le rôle du pharmacien est important dans la prévention des chutes médicamenteuses. Il informera ses patients de la présence dans leur traitement de médicaments pouvant favoriser une chute et accompagnera cette information de conseils permettant de réduire ou d’éviter la survenue de celle-ci.

La chute chez la personne âgée est une expérience fréquente, aux conséquences multiples, représentant un problème majeur en gériatrie. En dehors des conséquences traumatiques, les chutes sont souvent à l'origine d'une perte d'autonomie pouvant conduire à l'institutionnalisation. Les répercussions sont considérables en terme de santé publique et représentent un enjeu socio-économique important. L'importance du problème justifie la mise en place de mesures de prévention. Nous rapportons les expériences des consultations de la chute déjà mises en place à Dijon, Lille et Nantes et proposons la mise en place d'une telle consultation à l'Hôpital Intercommunal de Pornic. Un bilan complet doit être réalisé, incluant l'interrogatoire et l'examen clinique, la recherche des facteurs de risque, une évaluation de l'environnement du patient et les tests spécifiques d'évaluation de l'équilibre et de la marche.

Contexte : En France, l'espérance de vie sans incapacités stagne ces dernières années. La chute est un évènement grave et fréquent chez les personnes âgées. Elle est un des principaux modes d'entrée dans la dépendance. Souvent banalisée par les patients, la chute n'entraîne pas un bilan systématique alors que l'identification des facteurs de risques en amont permettrait d'éviter les récidives et les complications. Objectif : L'objectif de notre étude est d'identifier les différents facteurs de risques médicaux et médicamenteux de chutes retrouvés chez les patients hospitalisés en gériatrie suite à une chute. Méthode : Une étude descriptive, rétrospective et monocentrique a été menée du I er janvier 2016 au 30 septembre 2016 dans le service de court séjour gériatrique au CHRU de Besançon. Résultats : Quatre-vingt-sept patients âgés en moyenne de 88, 1 +/-5,3 ans ont été inclus. Les cinq facteurs de risques médicaux les plus présents sont : un âge de plus de 80 ans (94,2 %), une polymédication (77,0 %), un déclin cognitif (77,0 %), le sexe féminin (66,7 %) et la présence d'une arthrose des membres inférieurs et/ou du rachis (51, 7 %). En moyenne les patients cumulaient 7, 1 facteurs de risques médicaux. Trente-six pour cent des médicaments prescrits sont des médicaments majorant les risques de chutes. En moyenne, il en a été prescrit 2,66 +/- 1,72 pour chaque patient. Les principales classes médicamenteuses retrouvées sont les antidépresseurs (16 %), les diurétiques (13 %), les benzodiazépines (12 %), les inhibiteurs de l'enzyme de conversion (9 %) et les antidiabétiques (9 %). Conclusion : La chute doit être considérée comme un symptôme et non une fatalité liée au vieillissement. Elle ne doit pas être banalisée, sa prévention est essentielle. Médecins et pharmaciens ont, dans un travail de pluridisciplinarité, un rôle capital à jouer dans l'identification des facteurs de risques de chutes du sujet âgé et la possible modification de ceux-ci. La prévention de la chute est un enjeu en matière de santé publique.
