Le nombre d'interruptions volontaires de grossesse est en constante progression, particulièrement chez les mineures. Pourtant la contraception a connu une large diffusion depuis 30 ans. Notre étude a eu pour objectif de faire un état des lieux en 2006 des connaissances des médecins généralistes et de leurs jeunes patients en matière de contraception et de situations à risque de grossesse non désirée. Notre hypothèse est un manque de connaissance des jeunes en matière de contraception et de conduite à tenir en cas de rapport non protégé, de rupture du préservatif ou d'oubli de pilule associé à un manque d'information des médecins généralistes concernant ces questions. Nous avons mené une enquête prospective basée sur des questionnaires patients adressés à 300 médecins généralistes exerçant à Paris. Chaque médecin devait répondre à un questionnaire de pratiques puis inclure les deux prochains garçons et les deux prochaines filles âgés de 15 à 25 ans se présentant au cabinet médical et n'ayant pas de désir d'enfant au moment de l'enquête. Le taux de réponse de 10,7% nous a permis d'étudier les questionnaires de 32 médecins généralistes et de 128 patients. L'analyse des résultats a révélé un manque de connaissance des jeunes en matière de contraception avec une ignorance plus marquée encore chez les garçons. En cas d'oubli de pilule contraceptive, si le délai de rattrapage est dépassé, 35,9% des garçons ont déclaré ne rien faire ou ne pas savoir quoi faire et seul 10,9% des filles ont pensé à utiliser la contraception d'urgence orale. Les médecins généralistes ont mieux répondu que leurs patients mais ont également montré de nombreuses lacunes. En conclusion, les connaissances des jeunes sont insuffisantes pour éviter les grossesses non désirées. Nous proposons d'informer les jeunes plus précocement, sans distinction de sexe et de systématiser l'information par un contenu simplifié et répété.
