Introduction : Une bonne compétence communication est essentielle dans l'exercice médical. Elle améliore la santé physique et mentale des patients. Elle améliore la santé mentale des médecins et diminue le risque judiciaire. Le besoin de formation des médecins en communication est largement reconnu. La Faculté de Médecine de Tours propose un enseignement de la relation médecin-patient par jeux de rôles à ses étudiants. L'objectif de cette étude était de vérifier si cet enseignement améliore la compétence communication des étudiants en médecine qui y participent. Matériel et Méthode : C'était une étude observationnelle d'une cohorte d'étudiants de Diplôme de Formation Générale en Sciences Médicales 3 tirés au sort parmi les participants à l'enseignement en 2017. Leur compétence communication a été comparée au début et à la fin de l'enseignement, par l'échelle de Calgary-Cambridge simplifiée. Résultats : 34 étudiants ont été tirés au sort, 27 ont été évalués. At the start of studies on health communication, scholars were primarily concerned with showing the ethical implications of a new approach to care and with collecting evidence to demonstrate its greater effectiveness as opposed to the paternalistic and mechanistic paradigms. Well into the second decade of the 21st century, different issues need to be addressed. Aging populations and the spread of chronic diseases are challenging the sustainability of health care systems worldwide; increased awareness of health issues among the population and greater citizen participation seem to threaten clinicians’ authority. In this new scenario, it is acknowledged that the quality of verbal communication plays a crucial role, but it is still not clear how it impacts on the outcomes of care, which are its constitutive components and how it interacts with the institutional, cultural and social context of interactions. This book suggests that the time is ripe for a fresh start in health communication studies. As Debra Roter points out in her foreword, this proposal “is ambitious in attempting to integrate perspectives derived from pragmatics and argumentation theory with those derived from quantitative methods of medical interaction analysis and its prediction of outcomes”. On the other hand, as Giovanni Gobber explains in his foreword, “health communication can profit from an application of a performance-oriented linguistic analysis that pays attention to the role of the various relevant context factors in speech events related to specific activity types”. In this way, the open questions regarding communication in medical encounters are considered under a new light. Objectif : décrire les moyens de formation à la communication existants dans le domaine de la santé, les compétences communicationnelles acquises par ces moyens de formation ainsi que les méthodes d'évaluation des enseignements, afin de cibler les meilleures pratiques et de les utiliser pour un enseignement à la communication destiné aux externes en médecine (CORPEX). Méthode : revue narrative de la littérature des enseignements à la communication pour les étudiants en santé ainsi que des compétences acquises lors de ces enseignements. Inclusion de 24 articles actuellement publiées sur 1219 sélectionnés dans 5 bases de recherche différentes (MEDLINE, PubMed sans MEDLINE, CINAHL, EMBASE, ERIC). Résultats et discussion : la communication en santé s'enseigne et a prouvé son efficacité auprès des étudiants en santé. Les enseignements efficaces mêlent les techniques d'enseignement théoriques, pratiques (entretiens simulés) associés à une supervision individuelle utilisant la vidéo, ou réalisée par un médecin sénior entrainé. Les grilles d'évaluations sont trop nombreuses et évaluent les mêmes compétences, témoin d'un manque d'homogénéité des enseignements. Les compétences communicationnelles les plus difficiles à acquérir sont la capacité d'écoute du patient et l'empathie. Conclusion : l'enseignement de la communication aux étudiants en médecine est crucial. Il doit être de qualité, proposant plusieurs dispositifs pédagogiques (théoriques et pratiques), bien encadré (supervision) par des professionnels formés. Il doit être reproductible et évaluable facilement par des grilles validées selon les compétences que l'on souhaite évaluer.
