Book Description
Excerpt from Lectures on the Pathology and Treatment of Lateral and Other Forms: Or Curvature of the Spine Four unpublished lectures have, however, been incorporated, with the view of rendering the subject more complete in all its details. The entire series has been carefully revised, and such additions made as experience enabled me to add. In the year 1854 I delivered a special course of lectures on orthopaedic surgery at the Grosvenor Place School of Medicine and it was my custom annually to give a few lectures on orthopaedic subjects, in the course of surgical lectures which I delivered, first in conjunction with the late Mr. George Pilcher, and subsequently with Mr. Spencer Wells. Excerpt: ...as I could have anticipated, the back certainly yielded quickly, up to a certain point, but the difficulty that presents itself is the great sense of weakness that follows, together with the necessity for the most complete support during the time the ligaments are gaining strength; so that I cannot perceive that any advantage is gained by this proceeding, as I believe any case that admits of relief or cure cau be quite as quickly restored without the operation as with it." "On the Nature and Treatment of Deformities," by R. W. Tamplin, London, 1846. The theory of M. Gue"rin, so ably and so forcibly advocated by its author, at one time impressed me with its importance, and with a desire to put it to the test of practical experience. I, therefore, in the year 1856, selected a case, a girl under my care at the hospital, in whom there was an unusual degree of prominence and tension of the spinal muscles, in the lumbar region on the convexity of the curve; in the dorsal region also the muscles on the convexity of the curve were tense and prominent. A drawing from a cast taken of this case is represented in Fig. 31. FIG. 31. Cage of double lateral curvature, showing in lumbar region unusual prominence and tension of the spinal museles, which I divided subcutaneouslv. My late colleague, Mr. Lonsdale, concurred with me in the opinion that this was a fair case for testing M. Guerin's operation, and therefore I divided subcutaneously the muscles on the convexity of the lumbar curve, making a deep and free division. Immediately after the operation two or three fingers could CONDITION OF MUSCLES--GAY'S OBSERVATIONS. 137 easily be placed in the gap between the divided extremities of the muscles.
