Avec la médicalisation de la fin de vie, de nouveaux questionnements éthiques apparaissent, obligeant notre société à légiférer. La création des directives anticipées permettrait d'ouvrir le dialogue entre le médecin traitant et son patient. Mais cette discussion semble freinée par les non-dits et le tabou entourant ce sujet. Finalement, parle-t-on de la mort en consultation ? Cette étude qualitative réalisée par entretiens semi-dirigés auprès de 11 patients et 10 médecins généralistes, met en parallèle les attentes et les perceptions de chacun à propos d'une discussion autour de la mort en médecine générale. Les résultats montrent que ce thème n'est que rarement abordé en consultation. Alors que patients et médecins s'accordent pour dire qu'il serait pertinent d'en parler, aucun ne prend l'initiative de ce sujet, craignant les réactions de l'autre. Patients et médecins partagent une vision commune de ce qu'est « une bonne mort ». Cette mort idéale se passe d'intervention médicale. Les médecins situent donc la question en dehors de leurs compétences. Les difficultés psychiques et neurocognitives à se projeter en situation de fin de vie sont un autre obstacle mis en avant par les sujets interrogés pour justifier les réserves qu'ils ont à anticiper cette discussion. Enfin, la charge émotionnelle et les mécanismes de défense mis en place restent un frein à l'abord du thème de la mort en consultation. Les médecins interrogés s'accordent pour dire que la communication est un enjeu majeur pour contourner ces problématiques et sont demandeurs de formations, conscients que la relation médecin-patient est un outil thérapeutique important. De leur côté, les patients sont demandeurs d'une médecine plus humaine, avec un médecin ayant une approche globale du patient. S'inspirant des soins palliatifs, l'usage de soins spirituels en médecine générale pourrait apporter un début de solution. En s'impliquant humainement, patients et soignants redonnent du sens aux situations de fin de vie, grâce entre autres à une meilleure gestion de leurs émotions et de celles d'autrui. Cette étude réaffirme l'importance thérapeutique de la relation médecin-patient, à fortiori dans les situations où le curatif laisse place aux soins palliatifs et d'accompagnement de la fin de vie.
