Objectif de l'étude : comparer le travail, l'accouchement et l'état du nouveau-né en fonction de l'âge maternel et du déroulement de la grossesse. - Méthode : Étude rétrospective cas-témoins au CHU de Besançon entre 1995 et 2003, comparant le déroulement de la grossesse, de l'accouchement et de l'état du nouveau-né, entre des primipares à partir de 40 ans [108 grossesses] et un groupe-témoin de primipares âgées entre 20 et 39 ans [200 grossesses]. - Résultats : Lorsque la grossesse est physiologique, à partir de 40 ans, le travail est significativement marqué par des anomalies du rythme cardiaque fœtal et une hyperthermie. Le risque de césarienne est accru. La grossesse est pathologique dans 45% des cas et parmi les 3 pathologies étudiées, seul le taux d'HTA [hypertension artérielle] est significativement plus élevé à partir de 40 ans. Le mode d'accouchement est significativement différent en cas de MAP [menace d'accouchement prématuré]. Aucune différence n'est relevée concernant le diabète gestationnel. L'âge de la mère n'influence pas l'état du nouveau-né à la naissance, quel que soit le contexte de la grossesse. - Conclusion : À partir de 40 ans, la grossesse de la primipare nécessite une surveillance rigoureuse, afin de dépister au plus tôt la moindre pathologie. Le facteur "âge" semble être pris en compte dans la conduite à tenir obstétricale
