Les effets indésirables médicamenteux chez les personnes âgées sont fréquents et souvent graves ; la qualité de la prescription est donc importante. Les médicaments potentiellement inappropriés sont définis comme des médicaments ayant un rapport bénéfice/risque défavorable ou une efficacité discutable alors même qu’il existe des alternatives thérapeutiques plus sûres. Nos travaux ont permis de proposer une liste de médicaments potentiellement inappropriés chez les personnes âgées de 75 ans et plus, adaptée à la pratique médicale française, et s’appuyant sur la méthode de consensus Delphi. Il s’agit d’une liste de 34 critères explicites, établie à des fins épidémiologiques. Les médicaments de cette liste sont à éviter d’une manière générale et dans la mesure du possible chez les personnes âgées. Compte tenu des alternatives thérapeutiques proposées, cette liste peut être aussi utilisée par les médecins comme une aide à la prescription. Cette liste ne peut toutefois être considérée comme un unique outil de lutte contre la iatrogénie médicamenteuse. En effet, nous avons constaté que les médicaments inappropriés n’étaient pas les principaux pourvoyeurs d’effets indésirables. En revanche, le nombre de médicaments prescrits est un facteur de risque de consommation de médicaments potentiellement inadaptés et de survenue d’effets indésirables chez les personnes âgées. Nous avons montré que l’hospitalisation dans un service de gériatrie permet de réduire d’une part le nombre total de médicaments et d’autre part le nombre de médicaments inappropriés. Cette étude montre aussi la différence de perception du risque d’effets indésirables entre les gériatres et les médecins traitants. La prise de conscience de la spécificité de la prescription chez la personne âgée doit passer par des formations répétées auprès des médecins et par une sensibilisation de la population. Enfin, dans la recherche épidémiologique comme dans l’aide à la prise en charge des patients âgés, il est nécessaire de combiner à la fois les approches objectives et subjectives pour mieux comprendre et réduire la iatrogénie médicamenteuse.
