INTRODUCTION La certification théorique des étudiants préparant le diplôme d'études spécialisées (DES) de médecine générale nécessite notamment la validation de 11 compétences professionnelles. Pour cela, les étudiants du Département de Médecine Générale (DMG) Paris Diderot rédigent des travaux nommés traces d'apprentissage, dont les récits de situations complexes et authentiques (RSCA) sont les plus élaborés. La compétence 9 « se préparer à l'exercice professionnel » couvre les apprentissages des internes dans le domaine de l'exercice professionnel et du professionnalisme médical (conjointement à la compétence 10 « actualiser et développer ses compétences »). L'objectif de notre travail est d'analyser les RSCA produits par les internes validant la compétence 9, afin de comprendre la manière dont ils perçoivent et assimilent le professionnalisme et l'exercice professionnel. METHODE La première partie du travail concerne l'analyse quantitative des traces validant les compétences 9 et 10 dans la base de données du DMG, qui référencie toutes les traces d'apprentissage validées par les étudiants. La base de données est explorée à travers plusieurs critères, notamment les mots-clefs descriptifs insérés par les étudiants. La deuxième partie du travail analyse un échantillon de 38 traces ayant validé la compétence 9, sélectionnées de manière aléatoire, uniquement sur lecture du titre. Nous avons analysé leurs structures sur le plan quantitatif, puis, sur le plan qualitatif, leurs contenus à l'aide d'une grille de lecture construite pour ce travail, afin de mettre à jour les thèmes généraux, situations cliniques, intervenants, et aspects du professionnalisme évoqués. RESULTATS La base de données est riche de plus de 10000 traces. Nous avons regroupés les mots-clefs redondants afin d'avoir une vision des tendances thématiques concernant les compétences 9 et 10. Sur le plan structurel, les traces sont très inégales dans la présentation générale (parties problématiques, conclusion et bibliographie particulièrement omises) mais aussi très variables, tant dans leurs globalités, (moyenne 2743 mots, écart-type 1527), que dans chacune de leurs parties. Les compétences accompagnant le plus souvent la compétence 9 sont les compétences 2 et 10 (« communiquer de façon efficiente avec le patient et/ou son entourage » et « actualiser et développer ses compétences »). Les traces sont majoritairement réalisées en stage ambulatoire. Les traces n'ayant pas de situation clinique pathologique comme thème principal sont les plus nombreuses (22 traces vs 16 traces). 21 traces comportent une situation complexe. Le professionnalisme est surtout évoqué à travers les engagements envers l'amélioration de la qualité des soins et de la sécurité du patient, les responsabilités professionnelles, la compétence professionnelle, et enfin l'honnêteté à l'égard du patient. La confidentialité et le secret médical sont aussi abordés. Les internes ne discutent quasiment pas certains thèmes majeurs du professionnalisme tels que le conflit d'intérêts, le système de santé, l'influence des entreprises pharmaceutiques ou d'assurances, ou encore les limites entre la vie professionnelle et personnelle du médecin. CONCLUSION Si tous les engagements envers le professionnalisme médical (selon la charte internationale) sont abordés, les internes les traitent de manière très inégale. La base de données gagnera à être complétée par un nouvel outil d'insertion des traces d'apprentissage. Les RSCA, où les problématiques et discussions sont plus intéressantes, se prêtent bien à l'apprentissage du professionnalisme, alors que les traces d'apprentissages sans situations complexes examinent certes, certains de ses aspects, mais sans réellement offrir la réflexivité permettant d'envisager la réelle assimilation du professionnalisme. D'autres méthodes d'enseignement et d'évaluation sont complémentaires aux RSCA afin d'envisager un enseignement diversifié du professionnalisme médical.

Jean-Baptiste Say (1767–1832) was one of the first great economists to have laid down the foundations of economic science. This book presents a translation which renders his works accessible to the English speaking world. Jean-Baptiste Say and Political Economy proposes a translation of a selection of eleven of Say’s texts.
