INTRODUCTION : Le déni de grossesse est généralement défini par le fait qu'une femme ne prenne pas conscience de sa grossesse au-delà de 20 semaines de gestation. Les mécanismes psychiques à l'œuvre dans le déni de grossesse sont méconnus. Des travaux suggèrent que les femmes ayant fait un déni de grossesse présenteraient souvent un fonctionnement limite (1). Les objectifs de notre travail étaient d'évaluer si le trouble de la personnalité borderline constitue un facteur de risque de déni de grossesse et de décrire les caractéristiques relevées dans des dossiers de déni de grossesse. MATERIEL ET METHODES : Il s'agissait d'une étude cas-témoins rétrospective réalisée à partir de dossiers de patientes suivies entre 2008 et 2020 par l'équipe de pédopsychiatrie périnatale PPUMMA. Les groupes de sujets ayant fait un déni de grossesse et de sujets témoins ont été appariés selon 4 critères : la tranche d'âge, les situations affective et socio-économique et la multiparité. En complément, une base de données a été constituée afin de relever les caractéristiques des patientes ayant fait un déni de grossesse. RESULTATS : Dans le cadre de l'étude cas-témoins nous avons apparié 29 cas avec 29 témoins. 10,3 % des cas avaient un diagnostic de trouble de la personnalité borderline contre 37,9 % des sujets témoins, Odds Ratio = 0,19 et un intervalle de confiance à 950/0 de [0.03 ; 0.88] (p = 0,03). Par ailleurs nous avons décrit les caractéristiques relevées dans 56 dossiers de patientes ayant fait un déni de grossesse. Nous avons observé dans cet échantillon un isolement affectif et une précarité socio-économique majeurs. CONCLUSION . D'après notre travail, le trouble de la personnalité borderline pourrait représenter un facteur protecteur du déni de grossesse, ce qui correspond à l'inverse de notre hypothèse de départ. Nous avons constaté que les femmes ayant fait un déni de grossesse présentaient fréquemment un fonctionnement en faux self avec peu de symptômes externalisés.
