Introduction : Le syndrome d’épuisement professionnel ou burn-out est un sujet tabou dans le milieu médical. Il se compose de trois dimensions : l’épuisement émotionnel, la dépersonnalisation et la perte de l’accomplissement personnel. Le but de ce travail était d’évaluer la prévalence du burn-out chez les médecins généralistes Picards, et d’identifier les facteurs associés au syndrome d’épuisement professionnel. Matériel et méthodes : Une étude épidémiologique transversale descriptive a été réalisée auprès des médecins généralistes picards à l’aide d’un questionnaire anonyme distribué lors de séances d’Enseignement Post Universitaire. Résultats : 201 questionnaires ont pu être exploités correspondant à environ 10% de la population totale des médecins généralistes Picards. La prévalence du burn-out chez les médecins généralistes Picards était de 54.7%. Les facteurs de risque étaient : un âge élevé, une activité professionnelle plus ancienne, être divorcé ou célibataire, un nombre d’actes médicaux quotidien élevé, un nombre de garde moindre, exercer dans le département de l’Oise, avoir une activité isolée, l’automédication, avoir le sentiment de négliger sa santé, être en Affection de Longue Durée, avoir déjà eu recours à un traitement psychotrope ou à une consultation spécialisée (psychiatrique ou psychologique) au cours de sa carrière et avoir déjà ressenti des idées suicidaires. Les facteurs protecteurs étaient d’avoir un secrétariat, un remplaçant et/ou des loisirs. Discussion-Conclusion : Cette étude montre que plus d’un médecin généraliste picard sur deux est concerné par le burn-out. Plusieurs facteurs ont été identifiés. Des mesures de préventions semblent indispensables.

INTRODUCTION : De nombreuses études témoignent d’un degré de burn out (BO) élevé dans la population des médecins généralistes. Le BO est caractérisé par une dépersonnalisation dans la relation à l’autre, un épuisement émotionnel et une diminution de l’accomplissement personnel. OBJECTIFS : Le but de cette étude était d’étudier la prévalence du BO des médecins généralistes (MG) Picards et d’évaluer le lien entre BO et densité en médecins généralistes. METHODE : Nous avons réalisé une étude descriptive transversale en 2009 à partir du questionnaire d’autoévaluation « Maslach Burn out Inventory » (MBI). Le questionnaire a été envoyé à un échantillon aléatoire de 840 médecins généralistes. RESULTATS : Le taux de réponses était de 45,8%. D’après le MBI, 33,0% des MG répondants entretiennent des attitudes négatives de dépersonnalisation vis à vis de leur patient, 20,3% ont un niveau d’épuisement émotionnel élevé, 32,2% ont un score élevé de BO dans la dimension accomplissement personnel. La prévalence du BO est de 56%. Un niveau de BO élevé dans les 3 dimensions est retrouvé chez 6% des MG Picards. Un lien a été mis en évidence entre densité faible en MG et épuisement émotionnel élevé. Toutefois la robustesse de ce résultat a été mise en cause. DISCUSSION : La prévalence du BO des MG en Picardie diffère peu de celle des autres régions pour lesquelles nous disposons d’études. L’épuisement émotionnel est plus faible que dans les autres régions. Le BO n’étant pas lié à la densité médicale, ces résultats sont rassurants pour la qualité de la relation médecin-malade en Picardie. CONCLUSION : Il serait intéressant de prolonger ce travail par une étude qualitative visant à déterminer quels sont d’après les médecins eux-mêmes les véritables causes de leur burn out.

Introduction : le burn out est un problème majeur de santé publique. Pourtant aucune structure de prévention efficace n'existe. Nous avons donc évalué la connaissance du burn out et des structures de prévention existantes par les médecins généralistes de Picardie. Nous avons repéré les besoins de ces derniers, et, les moyens les plus adaptés à leur prévention. Matériel et Méthode : une étude quantitative a été réalisée, sur une population de médecins généralistes picards, par le biais d'un questionnaire anonyme de 18 questions. Résultats : sur les 428 praticiens interrogés 75 ont répondus. Les répondants avaient le même profil sociodémographique que les médecins Français, toutes les tranches d'âge étaient représentées. Les structures de prévention n'étaient connues que de 17.8% des répondants, la moitié se sentait suffisamment informée. Les facteurs précipitants les plus cités étaient la quantité d'heures de travail, les problèmes de gestions du cabinet et le manque d'aide des autres médecins. Les facteurs protecteurs les plus cités étaient l'organisation du cabinet et les vacances. Le moyen de prévention le plus cité était la réalisation de FMC, le besoin de reconnaissance apparaissait également. Discussion : il apparait clairement que des moyens de préventions, notamment primaires, doivent être instaurés. La souffrance des médecins généralistes est importante et la position du médecin doit être réaffirmé. Conclusion : le burn out est un sujet complexe et multiple comme sa prévention. Il est donc nécessaire de poursuivre ce travail afin de déterminer une série de reforme pour la prévention des médecins. Le mal être est grand et nécessite d'être pris en charge.
