L’objectif de notre étude est d’évaluer l’intérêt d’un médecin d’accueil d’orientation et de circuit court pour réduire l’engorgement croissant du service d’Urgences du Centre Hospitalier de Valenciennes. L’étude est prospective sur 8 jours et porte sur 609 patients admis aux urgences. Les quatre premiers jours, l’IAO effectue seule l’accueil et le tri des patients. Les quatre autres jours, un MAO s’associe à l’IAO. Le MAO n’influence pas de façon significative la durée de passage aux urgences. 15 % des admissions ne rentrent pas dans le circuit classique des urgences et sont gérés par le MAO à l’accueil. Le temps de passage aux urgences dépend pour partie, du fait de la réalisation d’examens complémentaires. Les rôles du MAO doivent être mieux définis. La collaboration avec le secteur libéral et les services hospitaliers d’amont sont indispensables pour optimiser les champs d’action du MAO et diminuer l’engorgement des Urgences à terme.
