Introduction. La ventilation non invasive (VNI) est un mode de ventilation dont l'efficacité est démontrée. Elle a fait l'objet d'une conférence de consensus en 2006. Notre étude porte sur la corrélation entre ces recommandations et l'utilisation de la VNI par les urgentistes de Sens en extrahospitalier et au service d'accueil des urgences (SAU). Méthode. Etude du 1er janvier au 30 avril 2017. Les critères d'inclusion étaient : > 18 ans, instauration par un urgentiste de Sens, en extrahospitalier ou au sein du SAU. Pour chaque patient inclus, l'urgentiste devait remplir un questionnaire dont les données étaient complétées et comparées à celles des dossiers médicaux. Résultats. Notre population comprenait 35 patients. L'âge moyen était de 77,1 ans. La VNI a été instaurée pour 6 patients en extrahospitalier, pour 21 patients au SAU. Les principales indications étaient des exacerbations de BPCO, des OAP, des acidoses hypercapniques. Ces indications étaient à 65,7% en accord avec les recommandations de grade G1+. Dans aucun dossier médical la surveillance clinique n'était prescrite. Durant la première heure de VNI, la fréquence respiratoire était notée dans 62,9%, la gazométrie dans 51,4%. La VNI a été un succès pour 40% des patients. L'ensemble des patients a été hospitalisé. Conclusion. L'utilisation de la VNI par les urgentistes sénonais est en accord avec la conférence de consensus. Des axes de progression sont identifiés dont la surveillance. La création d'un protocole pourrait permettre d'homogénéiser les pratiques.
