L'utilisation du mélange oxygène-protoxyde d'azote (MEOPA) dans le cadre de la prise en charge de la douleur liée aux soins en HAD chez l'adulte en France en 2014 : étude préliminaire pour sa mise en place à l'HAD du littoral Boulogne-Montreuil dans le Nord Pas-de-Calais Contexte : Dans le contexte des 3 Modes de prise en charge Principaux (MP) assurés par la structure: Soins Palliatifs, Soins de Nursing Lourd, et Pansements Complexes, le personnel soignant de l'HAD du Littoral (HADL) est en permanence confronté à la nécessité de pratiquer certains gestes de soins grandement pou,-voyeurs de douleurs induites auprès des patients. Le recours à l'inhalation du MEOPA, un gaz aux propriétés antalgiques anxiolytiques à l'origine d'une sédation consciente est une stratégie thérapeutique sûre et efficace rapidement, particulièrement adaptée pour ce type de douleurs, fréquentes en HAD dont l'utilisation n'est pas encore généralisée en HAD. D'ailleurs, elle n'est pas encore développée à l'HADL. Objectifs : Réaliser une étude préliminaire pour sa mise en place à l'HADL : étude du produit MEOPA, de ses conditions de délivrance et d'administration par les professionnels de santé concernés à l'HADL, particulièrement pour améliorer la prise en charge de la douleur liée aux soins. Etude du coût lié à l'utilisation du produit dans le cadre de sa mise en place éventuelle à l'HADL. Méthodes : 1/Pour le MEOPA (produit, approvisionnement, stockage, transport, délivrance) : Analyse des données de la littérature jusqu'en 2014 complétée par des enquêtes de terrain via des entretiens téléphoniques auprès des principaux fournisseurs. 2/ Étude des besoins de l'HADL via une enquête de terrain menée sur 3 semaines consécutives par l'IDE coordonnatrice avec le recueil du nombre de patients «éligibles» au MEOPA 3/ L'organisation de l'administration générale du MEOPA à l'HADL : enquête de terrain menée auprès des médecins généralistes (MG) libéraux du bassin de Fruges via un questionnaire standardisé, visant à estimer leur intérêt pour le MEOPA et celui porté à sa formation pour en maîtriser son utilisation.4/ Calcul du coût et de la valorisation en fonction des MP à l'HADL. Résultats : 1/ Le MEOPA est une technique antalgique sûre, rapidement efficace, avec peu d'effets secondaires -entièrement réversibles- particulièrement appropriée pour la prise en charge des douleurs induites. 2/ Il reste un médicament réservé à l'usage professionnel, avec principalement un risque de mésusage/abus/dépendance. Pour sécuriser son utilisation hors des réserves hospitalières (autorisée depuis 2009), l'ANSM a prévu un Plan national de Gestion des Risques (PGR) avec un suivi de pharmacovigilance et addictovigilance, imposant des conditions de délivrance et administration dans un cadre précis. 3/ Sa mise en place est faisable à l'HADL et permettra de valoriser la prise en charge de la douleur des patients. La dépense lié à son approvisionnement et à la formation du personnel a un coût supportable pour l'HADL. Il pourra être coté dans la tarification T2A HAD sous le MP07 «Douleur», en tant que Mode de prise en charge Accessoire (MA) et permettre une revalorisation financière de la séquence de soins versée par la CPAM. 4/ 87,5% des MG du bassin de Fruges (14/16) ayant répondu au questionnaire sont intéressés par la mise en place du produit à l'HADL et par une formation visant à en maîtriser son utilisation dans ce contexte. Conclusion : Le MEOPA est un médicament de choix pour le soulagement des douleurs liées aux soins, particulièrement fréquentes à l'HADL, en raison de ses 3 MP (soins palliatifs, nursing lourds, pansements complexes). Sa mise en place éventuelle à l'HADL nécessite que tous les soignants soient formés (MG-IDE). Un protocole de soins «l'utilisation du MEOPA» permettra de garantir les conditions de sécurité nécessaires pour son utilisation au domicile du patient.
