Résumé: Introduction : Les conduites à risques des adolescents et les situations de mal-être sous-jacentes sont un véritable enjeu de santé publique dont la prévention et le dépistage relèvent de la médecine générale et nécessitent que l'adolescent puisse être reçu sans ses parents en consultation. L'objectif de cette étude est donc de faire le point sur les pratiques actuelles, comprendre les attentes et les difficultés rencontrées par les adolescents, les parents et les médecins généralistes à la mise en place de ce mode de consultation afin de proposer des moyens de le favoriser. Matériel et Méthode : Il s'agit de trois études réalisées dans le département des Hautes-Alpes. La première est une étude quantitative descriptive réalisée auprès d'adolescents de 15 à 18 ans scolarisés en lycée à l'aide de questionnaires. La seconde est une étude quantitative descriptive réalisée avec des médecins généralistes par questionnaire également, et la troisième et une étude qualitative par entretiens semi-dirigés téléphoniques auprès de parents d'adolescents de 15 à 18 ans. Résultats : 1102 lycéens de 15 à 18 ans, 63 médecins et 9 parents ont participé à ces études. Les résultats montrent que la consultation des adolescents sans leurs parents reste minoritaire, et que la problématique des conduites à risques n'est que peu abordée en consultation. A 18 ans, 1/4 des adolescents n'a encore jamais été reçu seul, alors que les médecins sont conscients de l'importance de ce mode de consultation et déclarent essayer de le favoriser, que les parents comprennent plutôt facilement l'intérêt de leur absence et que les adolescents eux-mêmes y sont favorables. Cette étude a également permis de montrer l'effet positif de ce mode de consultation sur la relation de confiance entre l'adolescent et son médecin et sur la fréquence de l'abord de problématiques spécifiques par le médecin. Conclusion : Il existe à la fois des pistes de réflexion et des moyens simples de favoriser ce mode de consultation au quotidien, et donc d'améliorer la prise en charge des adolescents.

L'adolescence est une période où le recours au médecin est le moins fréquent. Pourtant, les conduites à risque des jeunes peuvent compromettre leur santé à long terme. Cette enquête veut étudier le ressenti des adolescents face à leur médecin traitant et ce qu'ils attendent de la consultation. Il s'agit d'une étude transversale, descriptive, interrogeant les élèves de seconde, première et terminale de deux lycées marseillais en mai 2013. Un questionnaire écrit, anonyme, leur a été distribué en début de cours dans les classes. 201 questionnaires ont pu être analysés. 80,6 % des lycées ont consulté leur médecin traitant dans l'année et 11 % n'ont eu recours à aucun système de soins. Même si les adolescents ont confiance en leur médecin, ils ne parlent que rarement de leurs conduites à risque qu'ils soient accompagnés ou seuls. Seulement 16 % ont parlé du tabac, 11 % de l'alcool, 50 % du poids, 18,1 % de sexualité et 7,8 % des jeunes se sentant déprimés en ont discuté avec leur médecin. Les lycéens pensent que le médecin peut les aider pour leurs problèmes de santé physiques mais n'est pas là pour écouter leurs problèmes personnels. Ils sont, d'ailleurs, 75 % à avoir l'impression d'avoir tout dit à la fin de la consultation et 84,5 % déclarent ne pas confier leurs problèmes personnels. Le tiers ne semble pas influencer ses résultats et l'ensemble des lycéens n'a pas de préférence sur la présence de ce tiers. Par contre, les filles et les moins de 17 ans ont tendance à préférer être accompagné pendant la consultation. Les missions du médecin généraliste semblent devoir être rappelées aux jeunes. La majorité des adolescents consultant au moins une fois dans l'année, le médecin devrait profiter de cette consultation pour établir un plan de prise en charge comprenant le dépistage et a prévention des conduites risque en incitant le patient à revenir. D'autres études de plus grande envergure sur le ressentie des adolescents seraient nécessaires pour infirmer ou confirmer nos résultats, les conduites risque des jeunes représentant un réel problème de santé publique

Much orthodox economic theory is based on assumptions which are treated as self-evident: supply and demand are regarded as independent entities, the individual is assumed to be a rational agent who knows his interests and how to make decisions corresponding to them, and so on. But one has only to examine an economic transaction closely, as Pierre Bourdieu does here for the buying and selling of houses, to see that these abstract assumptions cannot explain what happens in reality. As Bourdieu shows, the market is constructed by the state, which can decide, for example, whether to promote private housing or collective provision. And the individuals involved in the transaction are immersed in symbolic constructions which constitute, in a strong sense, the value of houses, neighbourhoods and towns. The abstract and illusory nature of the assumptions of orthodox economic theory has been criticised by some economists, but Bourdieu argues that we must go further. Supply, demand, the market and even the buyer and seller are products of a process of social construction, and so-called ‘economic' processes can be adequately described only by calling on sociological methods. Instead of seeing the two disciplines in antagonistic terms, it is time to recognize that sociology and economics are in fact part of a single discipline, the object of which is the analysis of social facts, of which economic transactions are in the end merely one aspect. This brilliant study by the most original sociologist of post-war France will be essential reading for students and scholars of sociology, economics, anthropology and related disciplines.
