La physiopathologie du diabète de type 2 est caractérisée initialement par le développement d'une insulinorésistance, qui constitue la cible thérapeutique des agents antidiabétiques de la classe des thiazolidinediones. La pioglitazone, dernière représentante de cette classe, a été retirée du marché français en 2011. L'objectif de ce travail est d'évaluer les conséquences médico-économiques à un an de ce retrait. Une étude rétrospective, fondée sur les données de remboursements du régime général de l'assurance maladie en Franche Comté, a été réalisée sur un échantillon de 36 254 patients diabétiques. Cette étude n'indique pas d'effet délétère de l'arrêt de la pioglitazone sur la consommation de médicaments, le montant remboursé du traitement et des soins et le taux d'hospitalisation. Cependant, les variations entre les différentes stratégies thérapeutiques (monothérapie, bithérapie,...) peuvent laisser penser que, chez certains patients, la pioglitazone n'a pas été remplacée, ce qui peut induire un déséquilibre de leur diabète. En cas de substitution de la pioglitazone, les prescripteurs se sont préférentiellement orientés vers les médicaments de la classe des inhibiteurs de la DPP-4, les insulinosécréteurs et l'insuline. Une étude réalisée sur 54 patients diabétiques de type 2 suivis dans le service d'endocrinologie du CHU de Besançon ou par des médecins de ville, indique un déséquilibre glycémique significatif avec une augmentation de 0,5 point d'HbAlc. En parallèle, on observe une diminution significative du poids et de l'IMC, mais aussi une diminution significative du taux de HDL. L'arsenal thérapeutique du diabète de type 2 s'est développé ces dernières années. Cependant, depuis le retrait de la pioglitazone, seule la metformine agit sur l'insulinorésistance, composante à l'origine du développement et de l'évolution de la pathologie
