But : Dresser le profil des multipares de 40 ans et plus. Comparer leur pronostic néonatal et maternel à celui des multipares d'âge inférieur à 40 ans. Matériel et méthodes : Etude rétrospective, unicentrique au CHU de Rouen, comparant 172 multipares de 40 ans et plus à 156 multipares de 20 à 34 ans. La relation entre l'âge maternel et les évènements obstétricaux et néonataux a été étudiée en utilisant le test du Chi2, le test exact de Fisher ainsi que le test de Student. Résultats : L'âge > 40 ans a été associé à l'augmentation de pathologies préexistantes à la grossesse telle que l'hypertension artérielle, ainsi qu'à l'augmentation de certaines pathologies obstétricales parmi lesquelles les métrorragies du 1 er trimestre, les ruptures prématurées des membranes, les diabètes gestationnels et les naissances prématurées. Le recours à la césarienne est également plus fréquent. Conclusion : Les grossesses chez les femmes > 40 ans sont associées à l'augmentation des prévalences de certaines pathologies préexistantes à la grossesse, pathologies obstétricales et pathologies durant le travail.
