Le domaine de la fin de vie est complexe, et la science médicale y occupe une place centrale. Dans ce travail, nous nous placerons aux côtés du médecin pour déterminer son rapport au monde de la fin de vie, notamment en étudiant la genèse de sa décision. Nous utiliserons vingt-et-uns entretiens semi-directifs réalisés avec des médecins exerçant l'anesthésie-réanimation, les soins palliatifs et la médecine générale. À travers la multiplicité des éléments intervenant au cours du processus décisionnel et de la prise en charge d'un malade en fin de vie, la plus grande difficulté constatée par le médecin réside dans le facteur humain. Il peut être source d'un conflit potentialisé par la contre-productivité de l'encadrement juridique et par un défaut de réflexion et d'anticipation. Si on note l'ambiguïté de la loi et l'émergence difficile de la culture palliative, ces considérations sont les simples conséquences de choix sociétaux et politiques impactant le monde de la fin de vie. De la médicalisation de la mort à l'hypertrophie du principe d'autonomie, la mort est mise sous tutelle et le médecin sous contrainte. Cette problématique est d'ordre politique, mais les enjeux de la gestion du champ médical de la fin de vie relèvent aussi de la responsabilité de la profession médicale. On s'intéresse ici à un monde social qui nous invite au questionnement et qui semble trouver son essence dans les considérations philosophiques qui le sous-tendent.

Defending ethics in sport is vital in order to combat the problems of corruption, violence, drugs, extremism and other forms of discrimination it is currently facing. Sport reflects nothing more and nothing less than the societies in which it takes place. However, if sport is to continue to bring benefits for individuals and societies, it cannot afford to neglect its ethical values or ignore these scourges. The major role of the Council of Europe and the Enlarged Partial Agreement on Sport (EPAS) in addressing the new challenges to sports ethics was confirmed by the 11th Council of Europe Conference of Ministers responsible for Sport, held in Athens on 11 and 12 December 2008. A political impetus was given on 16 June 2010 by the Committee of Ministers, with the adoption of an updated version of the Code of Sports Ethics (Recommendation CM/Rec(2010)9), emphasising the requisite co-ordination between governments and sports organisations. The EPAS prepared the ministerial conference and stepped up its work in an international conference organised with the University of Rennes, which was attended by political leaders, athletes, researchers and officials from the voluntary sector. The key experiences described in the conference and the thoughts that it prompted are described in this publication. All the writers share the concern that the end result should be practical action - particularly in terms of the setting of standards - that falls within the remit of the EPAS and promotes the Council of Europe's core values.

Du site de l'éditeur: The study explores public health and education provision in Niamey, the capital of Niger, by merging the ethnographic study of public services with an anthropological analysis of the state and of local politics. Based on anthropological fieldwork carried out in a group of neighbourhoods in the periphery of Niamey, the study highlights the political dimensions of public service provision in a local arena where international development interventions and national plans meet local realities and where a wide range of actors and institutions, discourses, meanings, and practices are mobilized in the offering of and the regulation of access to public services. It focuses on the political, economic, and socio-cultural aspects of public service provision, too often hidden behind contemporary buzzwords of development such as community participation and decentralization that dominate global debates about education and healthcare in developing countries. The study brings forth the strategies of urban residents in dealing with daily challenges in the consolidation of service provision and in education and health-seeking trajectories. It shows that access to a satisfactory treatment of illness or a successful school career is premised on the ability to navigate on the medical and education markets, which are made up of a plurality of providers and of official and unofficial costs and transactions. Further, these public services engage different actors such as community committees, traditional chiefs, local associations, the municipality and elected municipal councillors, emergent leaders, NGOs, and international development aid. The study demonstrates that despite the uncertainty of state support in health and education provision and a widespread dissatisfaction with these public services, the image of the state as service provider is reproduced on a day by day basis through local efforts at securing public services.
