Aujourd'hui, la prise en charge du patient cancéreux doit s'assurer de la continuité des soins notamment pour les conséquences de sa maladie et du traitement. Cette thèse présente les moyens mis en place autour du patient pour améliorer l'accès aux soins dédiés aux effets indésirables de sa chimiothérapie. En milieu hospitalier comme en ville, des professionnels de santé accompagnent le patient sous chimiothérapie, ouvrent le dialogue et l'informent sur les effets indésirables. En tant que professionnel orienté vers le conseil, l'écoute et le soutien du patient, les pharmaciens d'officine ethospitaliers ont un rôle à jouer dans les soins de support dédiés aux effets indésirables.Afin d'évaluer la situation réelle en terme d'information sur les effets indésirables des chimiothérapies et de définir les besoins des patients, nous avons réalisé une première enquête auprès de patients hospitalisés dans un service ambulatoire. Cette enquête révèle que l'accès aux soins de support dédiés aux effets indésirables est satisfaisant à l'hôpital mais que le patient a besoin d'informations supplémentaires sur ces effets lors de son retour à domicile. Suite à ce constat, des fiches d'information sur les effets indésirables destinées aux patients ont été réalisées et nous avons également décidé de réaliser une deuxième enquêteauprès des pharmaciens d'officine, afin d'évaluer leurs connaissances et leur intérêt pour les soins de support dédiés aux effets indésirables des chimiothérapies. Conscients du soutien qu'ils peuvent apporter au patient de retour à son domicile, ils soulignent leur volonté de s'impliquer dans la prise en charge du patient cancéreux tout en insistant sur l'importance d'un relais hôpital-ville, actuellement insuffisant.Au travers des deux enquêtes réalisées, ce travail a permis de préciser les besoins des patients et de montrer l'intérêt des pharmaciens pour combler ces besoins. Ainsi,le pharmacien se retrouve parfaitement dans les problématiquesactuelles de relais ville hôpital mises en avant dans le plan cancer 2014-2017.
