All across the United States, individuals, families, communities, and health care systems are struggling to cope with substance use, misuse, and substance use disorders. Substance misuse and substance use disorders have devastating effects, disrupt the future plans of too many young people, and all too often, end lives prematurely and tragically. Substance misuse is a major public health challenge and a priority for our nation to address. The effects of substance use are cumulative and costly for our society, placing burdens on workplaces, the health care system, families, states, and communities. The Report discusses opportunities to bring substance use disorder treatment and mainstream health care systems into alignment so that they can address a person's overall health, rather than a substance misuse or a physical health condition alone or in isolation. The High Intensity Drug Trafficking Areas Program run by the Office of National Drug Control Policy (ONDCP) coordinates federal, state, and local efforts to combat drug trafficking in critical areas of the United States. ONDCP requires each high intensity drug trafficking area to (1) assess drug threats within its geographic area, (2) prepare strategies and initiatives to address these threats, (3) develop a proposed budget to accomplish its initiatives, and (4) prepare an annual report on its accomplishments. Each high intensity drug trafficking area is allowed to tailor its own program to address local needs. For example, one area has a substantially greater treatment and prevention component than the others because it is a major distribution center with a large number of hard-core drug abusers. The program has grown substantially since its inception, from the original five areas in 1990 to 20 in 1998. The number of headquarters staff overseeing the program has also grown--from one in 1991 to six today. ONDCP has made some progress in developing approaches to evaluate the program's effectiveness, but more work remains. Total budget authority for the program rose from $25 million in 1990 to about $162 million in 1998. Headquarters officials and three area directors said that the program has underscored the value of collocation and intelligence-sharing among federal, state, and local law enforcement officials.

Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

For nearly three decades, methadone hydrochloride has been the primary means of treating opiate addiction. Today, about 115,000 people receive such treatment, and thousands more have benefited from it in the past. Even though methadone's effectiveness has been well established, its use remains controversial, a fact reflected by the extensive regulation of its manufacturing, labeling, distribution, and use. The Food and Drug Administration regulates the safety and effectiveness of methadone, as it does for all drugs, and the Drug Enforcement Administration regulates it as a controlled substance.
