L'asthme est la maladie bronchopulmonaire la plus fréquente pendant l'enfance. Les mécanismes physiopathologiques de l'asthme impliquent une bronchoconstriction médiée par l'acéthylcholine, mais les indications spécifiques pour l'utilisation des anticholinergiques dans la prise en charge thérapeutique de l'enfant asthmatique est encore très discutée. Le but de cette étude était de déterminer la réponse bronchique à un atropinique : le Bromure d'Ipratropium ; chez 116 enfants asthmatiques âgés de 2 à 7 ans, pendant une période stable de leur maladie. L'Ipratropium entraîne une bronchodilatation chez 44 enfants (groupe I), les 72 autres enfants ne répondant pas constituant le groupe II. L'âge, le sexe, la capacité résiduelle fonctionnelle de base, les gaz du sang et l'existence d'un traitement de fond, comparés par analyse de covariance, ne sont pas significativement différents entre les deux groupes. Cependant les enfants qui ont un reflux gastro-oesophagien (RGO) ont une bronchodilatation plus marquée après inhalation d'Ipratropium comparés aux enfants qui n'ont pas de RGO. Ceci est en faveur d'un réflexe établie par le système cholinergique entre l'oesophage et les bronches dans les mécanismes physiopathologiques de l'asthme associés au RGO. Ces résultats démontrent que l'Ipratropium améliore la fonction respiratoire dans une large population de jeunes enfants asthmatiques, et représente un choix intéressant dans le traitement de fond des enfants asthmatiques, en particulier lorsque l'asthme est associé à un RGO.

This book is concerned with children's economic and social rights (sometimes referred to simply as children's social rights). Despite increased academic interest in both children's rights and socio-economic rights over the last two decades, children's social and economic rights remain a comparatively neglected area. This is particularly true with regard to the role of the courts in the enforcement of such social rights. Aoife Nolan's book remedies this omission, focussing on the circumstances in which the courts can and should give effect to the social and economic rights of children. The arguments put forward are located within the context of, and develop, long-standing debates in constitutional law, democratic theory and human rights.
