La fréquence du syndrome de Guillain Barré (SGB) est évaluée à 1,5 pour 100 000. Sa mortalité est de 5%. Mais la détresse respiratoire est en rapport avec une atteinte des muscles respiratoires responsable d'une hypoventilation alvéolaire et d'atélectasies qui sont souvent aggravées par des troubles de la déglutition. Actuellement, la ventilation mécanique au cours du syndrome de Guillain-Barré est indiquée lorsque sont constatés une détresse respiratoire, une anomalie des gaz du sang artériels, une réduction importante de la force des muscles respiratoires (quantifiée par mesure de la capacité vitale et des pressions inspiratoire et expiratoires maximales) ou des troubles majeurs de la déglutition. Le respect de ces critères aboutit à une intubation endotrachéale trop tardive, exposant le patient au risque de pneumopathie par inhalation ou surinfection de troubles de ventilation. Une étude récente réalisée dans le service de réanimation médicale de l'hôpital de Raymond Poincaré (AP-HP) et portant sur 270 patients a ainsi identifiée que 76% d'entre eux développaient une pneumopathie, qui était également la principale cause de décès (mortalité de 18%). Ces résultats nous ont conduits à préconiser que le recours à la ventilation mécanique soit plus précoce, plus particulièrement chez les patients susceptibles d'évoluer vers une insuffisance respiratoire aiguë, afin de prévenir la survenue de pneumopathie. Cette réflexion complète celles formulée lors d'une précédente étude de la même équipe qui a identifié les facteurs précoces prédictifs d'une détresse respiratoire aiguë.L'objectif de cette étude était d'évaluer, chez les patients atteints de SGB à haut risque de ventilation, l'impact sur la survenue de pneumopathies d'une prise en charge ventilatoire précoce dont la modalité (invasive ou non invasive) dépendrait de la présence de troubles de la déglutition.
