Introduction : Il existe en France une augmentation constante du nombre de passage aux urgences. Parmi toutes ces entrées aux urgences, un tiers concerne la traumatologie. Il a été montré par l’EPAC en 2017 que les traumatismes du membre inférieur et notamment le genou sont des motifs prépondérants de consultation dans le domaine de la traumatologie. Malgré cette constatation, cette discipline est peu enseignée en médecine d’urgence. Nous nous sommes donc intéressés à la prise en charge des traumatismes du genou aux urgences du CHU de Rouen, et notamment à l’apport des consultations post urgences dans ces traumatismes en particulier. Matériel et méthode : notre étude a eu lieu de Mars à Août 2023, sur les urgences des centres hospitaliers de Rouen et de Petit-Quevilly. Etaient inclus les patients majeurs consultant pour traumatisme isolé du genou. Les inclusions ont eu lieu de Mars à Juin 2023 de manière rétrospective. Il y avait ensuite un rappel téléphonique qui était effectué deux mois après le traumatisme (de Mai à Août) avec un questionnaire standardisé afin de connaître le devenir des patients. Résultats : 353 patients ont été inclus. Parmi eux, 112 ont été adressé en consultation post urgences (31.7%) et 241 n’ont pas eu de consultation de suivi. L’âge médian était de 32 ans, avec une prédominance masculine (59.5%). Deux tiers des patients étaient pris en charge par les internes du service. Nous avons montré qu’il existait une satisfaction globale bonne avec une médiane de 4/5, légèrement plus élevée dans le groupe CPU de manière significative. A deux mois il existait un taux de changement diagnostic égal à 20%. Les patients pris en charge en CPU pratiquaient plus de kinésithérapie de manière significative (OR : 1.7, p=0.0002) et avait plus eu recours à un orthopédiste (OR : 3.1, p= 0.001). Les facteurs identifiés de lésions du LCA dans cette étude étaient le sexe masculin, le fait d’être sportif en compétition et le traumatisme ayant eu lieu durant une activité sportive et/ou de loisir. Les facteurs influençant la prescription de CPU étaient une douleur élevée, la présence d’un épanchement et la prescription d’un arrêt de travail. Conclusion : Notre étude a montré une satisfaction globalement bonne des patients pris en charge sur cette période. La réévaluation précoce en CPU a permis une consultation facilitée avec un orthopédiste et une kinésithérapie plus fréquente. Il reste des améliorations à faire sur les performances diagnostiques aux urgences notamment concernant les ruptures du LCA. Ceci passe par une meilleure formation des urgentistes à la traumatologie, de nouveau outils comme des scores de dépistage et des protocoles en association avec orthopédistes et médecins du sport.
