Leur stratégie de prise en charge doit être économe des deniers publics mais efficace pour le patient. On se trouve face à une double obligation : la première est relative au patient qui se confie en urgence à un médecin : il attend de lui qu’il lui donne le meilleur traitement possible ; la deuxième obligation concerne la communauté des patients à venir : il faut conserver des moyens pour qu’ils puissent eux aussi bénéficier de la meilleure prise en charge possible, sans dépenser pour quelques-uns seulement ce qui devrait profiter à beaucoup. L'augmentation progressive du nombre de passages aux urgences depuis plusieurs années en France a amené les services d'accueil à se réorganiser afin de pouvoir évaluer rapidement les patients dès leur admission et identifier au sein de ce flux les urgences graves voire vitales. Ceci passe par la mise en place d'un tri dès l'admission par une infirmière organisatrice de l'accueil dédiée, à l'aide d'une échelle de tri validée. En France, la SFMU a développé l'échelle de la FRENCH pour effectuer ce tri. L'objectif de ce travail était d'évaluer l'impact de la présence d'une IOA appliquant l'échelle de tri de la FRENCH sur les délais de prise en charge médico-soignante des patients admis aux urgences. Cette étude observationnelle, rétrospective, monocentrique a été réalisée du 1er janvier 2016 au 31 décembre 2016 à l'aide de données extraites du logiciel du service des urgences de Vienne pour tous les patients admis aux urgences à l'exclusion de la pédiatrie. Le critère de jugement principal était la différence de délai médian avant la première prise en charge médico-soignante entre les patients triés et non triés. La durée totale de séjour ainsi que les caractéristiques générales des patients triés et non triés ont aussi été comparées. Au total 29180 patients ont été inclus, 21874 ont été triés, soit 74,96% et 7306 n'ont pas eu d'attribution de niveau de tri, soit 25,04%.L'âge moyen des patients était de 45 ans, la répartition hommes/femmes était homogène.
