Objectif: identifier et comprendre l'influence de l'âge dans la conception et la mise en œuvre du projet de soin des médecins généralistes, face à un patient âgé, atteint d'AEG à domicile. Matériel et méthode: une étude qualitative, avec entretiens semi-dirigés auprès de 11 médecins généralistes en Franche-Comté. L'analyse des verbatims a été réalisée par la méthode de théorisation ancrée, avec réalisation d'un double codage. Résultats : d'après les médecins généralistes, la définition de I' AEG parait peu adaptée chez la PA. Le vieillissement est perçu de façon négative. En pratique les médecins prescrivent plus de bilans biologiques, jugeant l'interrogatoire et la clinique moins fiables. Le recours aux examens d'imageries est moins fréquent devant les difficultés d'accès aux soins. Ils évitent le recours aux urgences dans la mesure du possible. L' AEG est perçu comme non réversible. La prise en charge repose sur le maintien à domicile et l'adaptation de l'environnement pour préserver la qualité de vie. L'abstention thérapeutique est préférée à l'acharnement thérapeutique. Enfin, le poids de la famille est plus important dans leur PEC. Discussion : l'influence de l'âge dans la conception et la mise en œuvre du projet de soin est majeure de par les divergences de définition et d'évolution de l' AEG. Elle semble liée aux représentations qu'ils ont du vieillissement. Enfin, l'attitude plus paternaliste du médecin face à ses patients âgés sera à l'origine d'une moins bonne prise en compte de la volonté du patient. Conclusion : il existe une réelle divergence entre la vision que les médecins ont de la prise en charge des patients âgés et leurs pratiques.
