Objectif : Un suivi gynécologique de bonne qualité est important pour la santé des femmes, mais elles sont nombreuses à y être réticentes, du fait de son caractère invasif et intrusif. Une harmonisation des pratiques de l'examen pelvien, et des indications à le pratiquer, est nécessaire. Le but de cette étude est d'identifier dans la littérature les situations dans lequel la réalisation d'un examen pelvien est nécessaire, au cours des consultations pour motifs gynécologiques les plus fréquents en médecine générale. Matériel et méthode : Une revue rapide de littérature a été réalisée, basée sur les recommandations de la Cochrane. Les bases de données Pubmed et Embase ont été interrogées en mai 2022. Les études publiées depuis 2010 ont été incluses ; la population étudiée était les femmes âgées de plus de 15 ans, non gravides, en population générale. Les études devaient être comparatives. La qualité méthodologique des études a été évaluée à l'aide de grilles standardisées. Résultats : L'examen pelvien d'une patiente asymptomatique lors d'une consultation « de routine » n'est pas indiqué, sauf si nécessité de dépistage de cancer du col de l'utérus. Les auto-prélèvements vaginaux peuvent être utilisés pour le dépistage d'IST, mais ne sont pas validés pour le dépistage du cancer du col de l'utérus en population générale. Une patiente présentant une symptomatologie pelvienne devrait avoir un examen exhaustif, orienté selon la clinique. Un TV normal ne doit pas conduire à un arrêt des explorations. Les auto-prélèvements vaginaux sont validés pour le diagnostic d'infection à C.trachomatis mais ne le sont pas pour le diagnostic d'infection à N.gonorrhea. L'interrogatoire et l'examen clinique ont de faibles performances pour le diagnostic de candidose vulvo-vaginale et de vaginose bactérienne. Conclusion : Cette thèse a permis de clarifier certains aspects de l'examen pelvien. De nouveaux travaux, de meilleure qualité méthodologique, sont nécessaires afin de pouvoir émettre des recommandations de bonne pratique.
