Introduction : La définition de la syncope est mal connue en France. Elle est fréquemment confondue avec le terme de "syncope à l'emporte-pièce". A l'inverse, bon nombre de pathologies étiquetées "malaises" rentrent dans le champ de sa définition. Sa prise en charge se fait souvent selon une stratégie d'évaluation du risque de gravité. Le rôle de la troponine dans cette stratégie est controversé. Objectifs : Evaluer l'incidence de la syncope aux urgences en France, les caractéristiques de sa prise en charge et les facteurs associés au dosage de troponine. Matériel et méthodes : Etude monocentrique rétrospective incluant tous les malaises pris en charge aux urgences du CHU de Toulouse entre janvier et juin 2017. Sélection des syncopes, relevé de leurs caractéristiques et analyse de leur association au dosage de troponine. Résultats : 1934 malaises ont été inclus parmi lesquels 20% sont des syncopes (389 dossiers). Elles représentent 0,8% des motifs de consultation aux urgences et 56% des malaises avec perte de connaissance. Classer les syncopes dans une catégorie (réflexe, hypotension orthostatique ou cardiaque) est impossible pour 50% des patients. Dans ce cas, la troponine paraît orienter la décision d'hospitaliser le patient. Son dosage, réalisé chez 28% des syncopes, semble influencé par l'âge>50 ans (OR = 2 ,73 [1,53-4,88]), la présence d'une douleur thoracique (OR = 9,04 [3,81-21,41]) et une anomalie à l'électrocardiogramme(OR= 4,53 [2,35-8,72]). Conclusion : La syncope représente 20% des malaises. L'évaluation du risque se base insuffisamment sur les paramètres cliniques recommandés au profit d'investigations complémentaires trop nombreuses, notamment la troponine dont le rôle dans cette indication reste à déterminer.
