Acute Kidney Injury (AKI) occurs in 2/3 of critically iii immunocompromised patients. Most of them experienced AKI stage 3, and renal replacement therapy is required in 26.9% to 45% of patients with hematological malignancies and 41% to 52% of kidney transplant recipients. Sepsis is the main contributor of AKI in the intensive tare unit (ICU) setting, but more than one cause of AKI is common. Patients should be investigated for some specific etiologies (tumor lysi! syndrome, cast nephropathy in myeloma patients) as they may benefit from preventive or therapeutic strategies that have changed the prognosis in the recent years. The occurrence of AKI is associated with an increased mortality, gradually with the severity of AKI, but the underlying etiology may influence the prognosis. AKI also reduces the ability to receive optimal chemotherapy and therefore decreases the likelihood of achieving complete remission. Renal recovery, a major challenge to avoid subsequent complications of chronic kidney disease, is highly variable and depends on the underlying etiology. Half of the critically III kidney transplant recipients experienced deterioration in renal function or are dialysis dependent after ICU discharge. The identification of independent risk factors associated with the non-recovery of renal function prompts the development and evaluation of strategies acting on modifiable risk factors. A strategy to reduce the duration and/or the severity of sepsis-related AKI should combine early ICU admission, treatment of hemodynamic failure and hypoxemia, withdrawal of nephrotoxic drugs, and early adequate antibiotic therapy.

Contexte : L'insuffisance rénale aiguë (IRA) concerne plus de la moitié des patients de réanimation. L'IRA est source de progression de maladie rénale chronique (MRC). Chez les patients de réanimation, les données sur le pronostique rénal en fonction des caractéristiques de l'IRA sont limitées. En se basant sur la durée de l'IRA quelques études retrouvent des résultats contrastés concernant ce pronostic rénal. De plus, il n'y pas de donnée sur le pronostic rénal selon le mécanisme physiopathologique principal de l'IRA survenue en réanimation. C'est pourquoi nous avons construit l'étude MAKI pour décrire le pronostic rénal à long terme après une IRA en réanimation en fonction de sa durée et de son mécanisme physiopathologique principal. Méthodes : MAKI est une étude multicentrique, prospective et observationnelle. Le pronostic rénal d'une cohorte de 860 patients hospitalisés en réanimation plus de 24h sera évalué par le critère composite Major Adverse Kidney Events (MAKE). Ce critère MAKE sera évalué à 3 et 12 mois après le début de l'IRA ou de l'admission en réanimation si aucune IRA n'est survenue. Les patients seront classés en 3 groupes : 1) Les patients sans IRA durant leur séjour, 2) les patients ayant eu une IRA transitoire (récupération dans les 48h après le début de l'IRA), 3) les patients ayant eu une IRA persistante durant leur séjour.
