L'absence de travail spontané, il existe un risque infectieux pour lequel il convient de déclencher l'accouchement dans les 48 heures. Cependant, la crainte d'un sur-risque de césarienne en cas de déclenchement sème le doute dans les esprits. Le but de cette étude est de comparer deux délais de déclenchement (plus ou moins de 36 heures) en taux de césariennes et en taux d'infections materno-fœtales (IMF). Materiels et méthodes : Il s'agit d'une étude de cohorte rétrospective descriptive et analytique portant sur 237 patientes de janvier à décembre 2014 à la maternité de Bordeaux. Nous avons comparé deux populations de patientes ayant une grossesse physiologique et ayant rompu les membranes à plus de 36 SA avec un prélèvement vaginal négatif. Un 3e groupe était analysé, correspondant aux patientes ayant un travail spontané. Résultats : Il n'y avait pas de différence en termes de taux de césariennes si les patientes étaient déclenchées à plus ou moins de 36 heures (8,8% versus 9,7%, p= 0,90). Il n'y avait pas non plus de différence de taux d'IMF selon le délai de déclenchement (32,4% versus 19,4%, p= 0,40). 90,9% des patientes avaient un travail spontané dans les 25 premières heures. Leur voie d'accouchement n'étaient pas différentes des patientes déclenchées. Le taux d'IMF chez ces patientes était semblable aux patientes ayant été déclenchées (22% versus 17%, p= 0,78%). Discussion et conclusion : Ainsi, le taux de césarienne et d'IMF ne varient pas en fonction du délai de déclenchement si celui-ci est inférieur à 48 heures. Sous conditions d'un col favorable, nous pourrions alors proposer un déclenchement immédiat aux femmes qui le désirent.

Objectif : Evaluation des nouvelles pratiques de prise en charge des patientes présentant une rupture des membranes à terme avant travail (RMTAT) à la maternité de l'Hôpital Couple Enfant de Grenoble. Matériels et méthodes : Etude rétrospective AVANT/APRES comparant l'ancien protocole aux nouvelles pratiques de service allongeant le délai d'expectative après une RMTAT. Quatre groupes ont été formés selon le délai de prise en charge (ancien et nouveau protocole) et le statut par rapport au Streptocoque B. Les critères de jugement étaient le taux de mise en travail spontanée, le mode de déclenchement du travail, la voie d'accouchement et le taux d'infections materno-foetales. Résultats : Nous avons inclus au total 205 patientes. Chez les femmes avec un portage négatif ou inconnu pour le Streptocoque B, une expectative prolongée était associée à une augmentation non significative du taux de travail spontané. Quel que soit le portage pour le Streptocoque B, aucunes différences significatives n'ont été mises en évidence concernant le taux de césarienne et le taux d'infection materno-foetales. Conclusion : Les issues semblent comparables entre l'ancien protocole et les nouvelles pratiques de service concernant la voie d'accouchement et le risque infectieux.

Bolokoli, khifad, tahara, tahoor, qudiin, irua, bondo, kuruna, negekorsigin, and kene-kene are a few of the terms used in local African languages to denote a set of cultural practices collectively known as female circumcision. Practiced in many countries across Africa and Asia, this ritual is hotly debated. Supporters regard it as a central coming-of-age ritual that ensures chastity and promotes fertility. Human rights groups denounce the procedure as barbaric. It is estimated that between 100 million and 130 million girls and women today have undergone forms of this genital surgery.
