Introduction : L'objectif de cette étude est de comparer le nombre de réhospitalisations pour décompensation cardiaque, dans l'année suivant leur sortie, chez des patients hospitalisés pour décompensation d'insuffisance cardiaque chronique, selon leur prise en charge ou non dans un centre de SSR. Matériel et méthode : L'étude réalisée est observationnelle descriptive, prospective sur la période d'avril 2014, et monocentrique (CH Cannes). Les critères d'inclusions étaient un score NYHA entre 2 et 3, un BNP ≥ 400pg/mL, et une ETT retrouvant des signes d'insuffisance cardiaque (à FEVG conservée ou non). Ces patients vont ensuite choisir ou non un suivi de 3 semaines dans un centre de SSR (Groupe Oxford versus Groupe Domicile). Le critère de jugement principal de l'étude est la recherche d'une diminution du nombre de réhospitalisations à 1 an dans le groupe Oxford. Les critères d'évaluation secondaires vont évaluer la qualité de vie (score NYHA et QOL) des patient, rechercher les facteurs prédictifs ou protecteurs d'une réhospitalisation, et la mortalité dans chaque groupe. Résultats : 37 patients ont été inclus, 17 dans le groupe Oxford et 20 dans le groupe Domicile. 13 patients dans le groupe Oxford ont été réhospitalisés à 1 an contre 17 (p=0.51). Un patient dans chaque groupe est décédé (p=0.91). Les scores NYHA et QOL à un an respectivement de 26,87±9,54 et 2±0,63 contre 56,81±14,24 (p=0.044) et 2,26±0,81 (p=0.3) dans les groupes Oxford et Domicile. Discussion : Aucune différence significative n'a pu être montré quant au nombre de réhospitalisations à un an ou sur la mortalité. En revanche, il existe une amélioration significative de la qualité de vie dans le groupe Oxford.

Les pathologies chroniques touchent un quart de la population française. Les pathologies cardiovasculaires sont la première cause de décès en France, leur morbidité est importante, nécessitant une prise en charge chronique des patients. L’éducation thérapeutique est un outil de réduction de morbi-mortalité ayant fait preuve de son efficacité dans plusieurs récentes études. Cette étude réalisée sous forme d’une enquête téléphonique, 9 à 12 mois après la fin des séances éducationnelles, a pour but d’évaluer l’impact des séances d’éducation thérapeutique en termes de modification de mode et de qualité de vie de patients ayant une pathologie cardiovasculaire et participé à des ateliers éducatifs. Les résultats évaluant les différents ateliers éducatifs sont dans l’ensemble satisfaisants. Il y a une bonne compréhension et une prise de conscience des patients entrainant une modification de leurs habitudes de vie et une amélioration de leur qualité de vie. Néanmoins, les résultats sont plus modestes concernant les ateliers d’aide au sevrage tabagique, sur le diabète et sur l’insuffisance cardiaque. L’éducation thérapeutique est donc un outil indispensable et indissociable de la prise en charge des patients porteurs de pathologies cardiovasculaires. Cette enquête a également permis de mettre en évidence des points faibles à améliorer dans certains ateliers éducatifs, et a affermi la volonté de maintenir la collaboration entre les deux Centres Hospitaliers.
