Objectif : La mortalité postopératoire de la chirurgie cardiaque pour une endocardite infectieuse (El) est élevée et la décision d'une intervention concomitante non liée à l'EI pendant la chirurgie reste débattue.Méthodes : Cette étude rétrospective a inclus 171 patients consécutifs adressés en chirurgie pour une El du cœur gauche. Nous avons exclu de l'analyse les patients avec une instabilité hémodynamique à l'admission. Les interventions chirurgicales concomitantes non liées à une El comprenaient un geste tricuspide fonctionnel (n=20), un pontage aorto-coronarien (PAC n=12), un remplacement de l'aorte ascendante (12) et une chirurgie aortique ou mitrale non liée à l'endocardite (n=5). Le critère d'évaluation principal était défini par le décès cardiovasculaire (CV) à un an.Résultats : Une chirurgie non liée à l'EI a été réalisée chez 45 (26%) patients et 126 (74%) ont subi une chirurgie cardiaque exclusive. Les patients ayant subi une chirurgie non liée à l'EI présentaient un Euroscore-II plus élevé, des signes d'insuffisance cardiaque plus fréquents à l'admission, une FEVG plus faible, une troponinémie plus élevée, plus de coronaropathie significative et une durée de clampage aortique plus longue. La mortalité CV à un an (n=17, 10%) était plus importante chez les patients ayant subi une chirurgie non liée à l'EI (20,0% contre 6,3%, P=0,02), en particulier chez ceux orientés vers un PAC ou une chirurgie tricuspide (30% contre 5,7%, P=0,003). Cette association reste significative par analyse multivariée après ajustement à l'EuroScore-II (OR=3,4 IC 95% 1,1-10,8).
