Cancer is the second leading cause of mortality worldwide. Over 10 million new patients are diagnosed with cancer annually with over 6 million associated deaths, representing roughly 12% of worldwide mortality. The occurrence of new cancer cases is expected to grow by about 70% over the next two decades and estimated to reach over 15 million new cases diagnosed annually by the end of year 2020. This rapid increase is due to both an aging and growing population, along with carcinogens, infections, genetic mutations, hormones, immune conditions, and the adoption of behavioral and dietary risk factors, such as smoking, unhealthy diet, physical inactivity, and environmental pollutants. The risk factors may act singly or in concert to cause mutation of normal cells. Many of these mutations alter the expression or activity of key gene products, causing unregulated cell division leading to cancer. Currently, the main cancer treatment modalities are surgery, radiation-based therapy, chemotherapy, gene therapy, and/or hormonal therapy, either singly or in combination. The most commonly used chemotherapy drugs are antimetabolites, DNA-interacting agents, antitubulin agents, hormones, and molecular targeting agents, all of which work to destroy cancerous cells or limit their proliferation. However, most cytotoxic drugs act on both cancerous and healthy cells and therefore elicit side effects such as hair loss, bone marrow suppression, drug resistance, gastrointestinal lesions, neurologic dysfunction, and cardiac toxicity. Consequently, development of new anticancer agents with higher efficacy, selectivity, and little or no side effects is an urgent goal.Natural products, especially phytochemicals, have been used to help mankind sustain health since the dawn of medicine. Phytotherapy (also called herbalism or herbal medicine) has provided remedies for ailments, including cancer, to the present day. Dietary phytochemicals have many built-in advantages over synthetic compounds due to their proven safety, low cost, and oral bioavailability. However, it is only recently that researchers have begun to elucidate the mode of action of plant-derived agents at the molecular, cellular, and tissue level. Many natural products have now been extensively researched, and numerous compounds have exhibited anticancer and other beneficial actions in modern controlled studies. Most anticancerous natural products interfere with the initiation, development, and progression of cancer by modulating various mechanisms including cellular proliferation, differentiation, apoptosis, angiogenesis, and metastasis.Extracts from Annona muricata (also known as graviola) are among a myriad of botanical products which have shown promising medicinal value. Studies have linked A. muricata-derived compounds to a variety of anticancer effects including cytotoxicity, induction of apoptosis, necrosis, and inhibition of proliferation on a variety of cancer cell lines, including breast, prostate, colorectal, lung, leukemia, renal, pancreatic, hepatic, oral, melanoma, cervical, and ovarian cancers. Moreover, all aerial parts of this plant, including the bark, fruit, leaves, root, and seeds, are used as natural medicines in the tropics. However, there is a need for more rigorous studies to establish safe and effective care regimes. This review summarizes the recent advances in the application and mechanisms of A. muricata extracts against several cancers both in vitro and in vivo.

Annona muricata L. is a medicinal plant, commonly known as Graviola, Soursop, or Guabana. Graviola is a tropical evergreen fruiting tree, which belongs to the Annonaceae family. All parts of the Graviola tree are known to be effective against various human diseases such as cancer and parasitic infections. In particular, Graviola leaves were found to be effective against cystitis, diabetes, headache, insomnia, and inflammation. In South America and Africa, Graviola leaves have been applied as an ethnomedicine to treat cancer. Besides its medicinal uses, Graviola fruits are used for development of various food products including beverages, candy, and syrups.These traditional pharmacological uses of Graviola have evoked an interest for further elucidation of its effective components. Among various constituents in Graviola, acetogenins have been applied for cancer therapy. Annonaceous acetogenins are composed of 35- or 37-carbon long-chain fatty acids that form the polyketide skeleton. These acetogenins, derived from seeds, leaves, and pericarp have cytotoxicity against carcinoma cells of prostate, pancreas, and breast. Despite their anticancer efficacy, some acetogenins in Graviola leaves are known to be highly cytotoxic. In addition, chronic consumption of Graviola juice containing annonaceous acetogenins aggravates cerebral tau phosphorylation in mice and contributes to development of neuropathological changes observed in Parkinson's disease. Another important attribute about Graviola is its high content of antioxidant compounds. Phenolic acids and flavonoid glycosides in Graviola leaves are known to have potent antioxidant activities. Recently, we have reported that the total phenolic acid and total flavonoid contents were present in the steam and ethanol extracts of Graviola leaves in high concentrations, which is associated with their free radical-scavenging effects. Additionally, the ethanol extract of Graviola leaves was found to upregulate the expression of superoxide dismutase-1 (SOD-1) and nuclear factor erythroid-2-related factor-2 (Nrf-2), which contributed to its reactive oxygen species-reducing activity.
