La prise en charge de l’asthme et de ses exacerbations ont fait l’objet de recommandations internationales, dont celles du GINA. Cependant, les hospitalisations pour asthme restent trop fréquentes. L’objectif de cette étude était d’identifier et caractériser, au sein d’une population de patients hospitalisés en pneumologie pour asthme aigu grave, ceux qui présentaient un ou plusieurs facteurs jugés améliorables. Il s’agissait également de déterminer, sur un suivi à long terme, lesquels de ces patients étaient ré-hospitalisés. Cette étude prospective a été menée auprès de 169 patients asthmatiques adultes hospitalisés en pneumologie pour asthme aigu grave. Ces patients ont été classés en 2 groupes comparatifs, en fonction de la présence ou non de facteurs améliorables. Les patients ont ensuite bénéficié d’un contact téléphonique avec interrogatoire, réalisé 5 ans après l’hospitalisation initiale, afin de distinguer les patients ré-hospitalisés et leurs caractéristiques. La majorité des patients (83 %) avaient au moins un facteur améliorable, et ne bénéficiaient donc pas d’une prise en charge optimale selon les recommandations. Ces patients A étaient plus jeunes, davantage exposés aux risques, avec un asthme moins sévère. Ils étaient moins suivis, se surveillaient moins. Parmi les patients contactés à 5 ans, 52 % ont été ré-hospitalisés au moins une fois pour asthme. Ces patients ré-hospitalisés étaient plus nombreux à présenter un asthme sévère, étaient mieux suivis et se surveillaient mieux. 79 % des patients avec un asthme sévère contactés à 5 ans ont été ré-hospitalisés. Ainsi, malgré les recommandations internationales, il existe encore de nombreux patients pour lesquels certains aspects de la prise en charge sont améliorables, et qui nécessitent une hospitalisation. En outre, trop de patients sont sujets à des hospitalisations répétées pour asthme. Il est donc essentiel d’identifier pour chaque patient les défaillances de prise en charge et d’éducation afin de les corriger, et parvenir à prévenir la survenue des ré-hospitalisations, qui mettent en péril la vie du patient et sont responsables d’un surcoût
