La précarité ne cesse d'augmenter en France. Les études récentes prouvent que les personnes en situation de précarité se considèrent en moins bonne santé que la population générale et que l'accès aux soins leur est plus difficile. L'objectif principal était de déterminer les priorités de santé des personnes en situation de précarité, de mieux connaître leur représentation de la santé et les facteurs pouvant faire renoncer aux soins. Une étude qualitative a été réalisée avec seize entretiens semi dirigés auprès de résidents de Centres d'Hébergement et de Réinsertion Sociale, et d'usagers d'accueils de jour. Une analyse thématique des données par double codage a été effectuée. On constatait une primauté de la santé psychique sur la santé physique, pour permettre de supporter les conditions de vie. Le logement, l'emploi et l'alimentation étaient prioritaires sur la santé. Les conditions de vie étaient un frein à l'accès aux soins, de même que la complexité du système de soins. Les addictions prenaient parfois le pas sur la santé. Les personnes en situation de précarité étaient peu enclines à se soigner de peur d'être stigmatisées, ou par crainte de découvrir une maladie. L'obtention d'un logement était une étape importante pour parvenir aux soins. Les préoccupations de survie étant assurées, les personnes en situation de précarité pouvaient faire les démarches pour obtenir une couverture maladie et initier une démarche de soins. Le regroupement spatial et temporel de professionnels de santé et sociaux dans des centres de dépistage et la Couverture Maladie Universelle étaient des mesures appréciées. Une relation de confiance était attendue chez les professionnels de santé. Une plus grande proximité était demandée. Une meilleure connaissance des représentations de la santé et des priorités des personnes en situation de précarité permet une meilleure prise en charge.
