Failings in the care provided to people as they approach the end of their life can result in needless pain and suffering. In Dying without dignity, the Parliamentary and Health Service Ombudsman (http://www.ombudsman.org.uk/__data/assets/pdf_file/0019/32167/Dying_without_dignity_report.pdf) identified systemic issues affecting the quality and delivery of end of life care and used case studies to show how patients - and those close to them -had been affected by poor care. We welcome the report, which concluded that the NHS needed to improve in a number of areas and recommended that the report's findings be used to inform a new approach to end of life care. Witnesses told us that they were aware of the issues raised in the report and that the focus must now be on implementing change. We agree. We want good quality, timely and compassionate care to be available to all those who need it at the end of their life. In this Report, we outline the key areas in which we expect to see significant and fast-paced improvements. These are: (i) culture, behaviour and training; (ii) the provision of integrated, 24/7 palliative and End of Life Care (EOLC) services; and (iii) leadership and commissioning.

This report looks at the state of end of life care since the independent Review of the Liverpool Care Pathway, chaired by Baroness Neuberger. It finds great variation in quality and practice across both acute and community settings. It makes a number of recommendations for improvement, and in particular strongly recommends that social care should be free at the end of life. Other conclusions included that: all clinicians and providers who may care for people at the end of life should be aware of the Five Priorities of Care but in light of the variation in practice a senior named person in each NHS Trust be given responsibility for monitoring how end of life care is being delivered within their organisation; expertise should be more equitably available to people with a non-cancer diagnosis, older people and those with dementia; all staff who provide palliative and end of life care to people with life limiting conditions should receive training in advance care planning, including the different models and forms that are available and their legal status; most people who express a preference would like to die at home but that is made more difficult by the shortfall in community nurses and specialist outreach palliative care; sustainable, long term funding for the hospice sector also needs to be addressed as part of the Government's response to the Palliative Care Funding Review; and bereavement support for families should also be consistently included as part of end of life care

Honour based violence and abuse manifests itself in different forms, and this book offers a comprehensive understanding of this phenomenon. This book argues that the limits of honour crimes must be defined more widely so that they include conducts and behaviours that originate from the patriarchal notion of honour, such as honour based oppression and breast ironing. The book provides a critical analysis and synthesis of the law in England and Wales and in the international human rights sphere. The relevant domestic legislation and cases are examined to reflect on whether adequate protection is provided for the victims and potential victims of honour based violence and abuse. Since honour based violence is a violation of human rights, the relevant international human rights law is examined to illustrate the perception of such crimes in the international arena. The effectiveness of any remedy for victims of honour based violence and abuse depends on its capability to change deep rooted behaviours in communities with honour based patriarchal values. This book argues that the law does not provide the effective impact required, in part due to patriarchal structures, and that more efforts should be dedicated to changes in education. It is held that there is a need for an educational programme that is especially designed to tackle violence and promote gender equality. The book will be essential reading for academics, researchers and policy-makers working in the areas of Human Rights Law, Criminal Law and Gender Studies.

Physician-Assisted Death is the eleventh volume of Biomedical Ethics Reviews. We, the editors, are pleased with the response to the series over the years and, as a result, are happy to continue into a second decade with the same general purpose and zeal. As in the past, contributors to projected volumes have been asked to summarize the nature of the literature, the prevailing attitudes and arguments, and then to advance the discussion in some way by staking out and arguing forcefully for some basic position on the topic targeted for discussion. For the present volume on Physician-Assisted Death, we felt it wise to enlist the services of a guest editor, Dr. Gregg A. Kasting, a practicing physician with extensive clinical knowledge of the various problems and issues encountered in discussing physician assisted death. Dr. Kasting is also our student and just completing a graduate degree in philosophy with a specialty in biomedical ethics here at Georgia State University. Apart from a keen interest in the topic, Dr. Kasting has published good work in the area and has, in our opinion, done an excellent job in taking on the lion's share of editing this well-balanced and probing set of essays. We hope you will agree that this volume significantly advances the level of discussion on physician-assisted euthanasia.
