Le remplacement est une forme de pratique en Médecine Générale, exercée par différents profils : internes, médecins thésés ou non, titulaires d'une licence de remplacement. Tout comme le profil des remplaçants, les offres sur le terrain sont très diversifiées. Nous avons souhaité analyser les différents facteurs influençant le choix du terrain de remplacement dans les départements de la Marne et des Ardennes. Pour cela, nous avons réalisé une étude quantitative descriptive par le biais d'un questionnaire en ligne. L'analyse statistique en sous population des 70 réponses exploitables nous a permis d'établir que certains facteurs (distance par rapport au domicile principal, aménagement de l'agenda, visites à domicile...) étaient déterminants. Dans un second temps, nous nous sommes intéressés aux caractéristiques (sexe, âge, statut, lieu de vie...) des répondants afin d'étudier les éventuelles interactions avec ces facteurs. Finalement, cette étude cherche à mettre en lumière les facteurs qui ont une influence, positive ou négative, dans le choix du terrain de remplacement en Médecine Générale.

Introduction : Peu de travaux se sont intéressés aux exigences et préoccupations des médecins généralistes remplaçants. Notre étude avait pour objectif de mesurer l'importance des critères influençant le choix d'un remplacement et d’en établir une hiérarchie. Matériel et Méthode : Un questionnaire en ligne a été diffusé aux médecins remplaçants inscrits sur le site du conseil de l’Ordre de la Somme et aux adhérents de deux syndicats. 102 réponses ont été obtenues. Résultats : Les cinq éléments les plus importants figurant dans la hiérarchie des critères étaient, par ordre décroissant : les critères géographiques, l’aménagement de l’emploi du temps, l’aspect financier, l’organisation de l’activité du remplacé et enfin l’entente avec ce dernier. Parmi ces cinq critères majeurs, la proximité du domicile, la fréquence des remplacements et les périodes d’activité proposées, le pourcentage de rétrocession et le taux d’activité journalier, le nombre de consultations et la priorité aux consultations sur rendez-vous, une relation de confiance ont été identifiés comme des facteurs influençant la recherche d’un remplacement. Discussion : Les hiérarchies de critères retrouvées dans la littérature étaient différentes de celle de notre étude. Néanmoins, il existait un réel consensus sur l’importance que revêtaient l’aspect financier et les critères géographiques sur le choix d’un remplacement. La classification variait selon l’âge des médecins remplaçants, et l’aménagement de l’emploi du temps était le critère le plus impacté par l’organisation familiale. Conclusion : Notre étude a montré que le choix des remplacements se faisait selon une hiérarchie de critères bien établie et variable selon le profil, au premier rang desquels figure la proximité géographique.
