La Haute Autorité de Santé propose des recommandations de bonne pratique sur l'accompagnement familial et le suivi de l'enfant sourd. Les services d'ORL pédiatrique de la Timone et du CAMSP DA de Salvator tentent de répondre à ces recommandations. Afin de dresser un état des lieux et pour améliorer leur pratique, des témoignages parentaux sur le parcours de soin de leur enfant sourd ont été recueillis. Pour cela, nous avons proposé un questionnaire d'évaluation de la prise en charge étudiant les aspects quantitatifs et qualitatifs. Les résultats obtenus montrent globalement une bonne satisfaction, notamment en termes d'accueil dans le service, de délais d'attente, d'informations reçues et comprises, de qualité des prises en charge et des professionnels les assurant. On note cependant que les informations données sur certains types d'appareillages et modes de prises en charge pourraient être améliorées. L'analyse qualitative a permis d'observer l'évolution des attentes avec l'avancement de la prise en charge. Cette évolution étant en lien avec le cheminement psychique des familles quant à l'acceptation du handicap de leur enfant. Elle a également permis de faire ressortir l'importance de l'écoute et de la disponibilité des professionnels, notamment les orthophonistes. Il ressort aussi que les informations dispensées doivent bénéficier d'un ancrage, pouvant passer par le support écrit, de redondance et d'une approche multidimensionnelle: livret d'informations, groupe de parole, analyse de films... Cette étude a montré la bonne adéquation des familles à la prise en charge proposée. Elle a également ouvert des perspectives de travail comme la passation de nouveaux questionnaires, la création et la mise en place de supports écrits et le développement de l'information sur l'implant cochléaire.

Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)
