Objectif : Etudier les pratiques de dépistage du diabète gestationnel (DG) après mise en place d’un nouveau protocole en 2005. Matériel et méthodes : Etude rétrospective de type avant après à partir de 85 dossiers de femmes ayant accouché à la maternité de niveau III du CHU de Grenoble entre le 1er février et le 31 juillet 2004 d’une part et entre le 1er février et le 31 juillet 2006 d’autre part. Résultats : La prévalence du DG était de 5% en 2004 et de 4,7% en 2006. Les femmes suivies en 2006 avaient significativement plus de facteurs de risque du DG et plus de dépistage orienté. Il y avait une diminution de l’âge gestationnel moyen au test de dépistage (test O’Sullivan), de l’âge gestationnel moyen au test diagnostic (HGPO 100g) et du délai entre les deux tests mais les différences observées n’étaient pas significatives. La prise en charge endocrinologique et diététique était plus rapide mais ce n’était pas significatif. Il n’y avait pas de différences significatives concernant le retentissement fœtal à la dernière échographie et la morbidité materno-fœtale associée au diabète gestationnel. Conclusion : Le protocole semblait être utilisé puisque nous avons observé un dépistage et un diagnostic plus précoces ainsi que des délais plus courts entre les deux tests, même si les différences observées n’étaient pas significatives. De plus, nous avons pu observer un manque de dépistage systématique entre 24 et 28 SA ainsi qu’un retard de dépistage des femmes ayant un ou plusieurs facteurs de risque du DG. Malgré une prise en charge endocrinologique et diététique plus rapide, il y avait moins de consultations diététiques, les prises de poids en fin de grossesse étaient plus importantes et le diabète difficilement équilibré. Ceci nous permettait donc d’insister sur l’importance du suivi diététique car c’est un point essentiel dans la prise en charge du diabète gestationnel. A plus grande échelle, ce suivi diététique serait le point de départ pour une action de prévention dans le risque de survenue d’un diabète de type 2 chez les mères et d’obésité chez leurs enfants.

Matériel et méthodes : Etude rétrospective de type « avant-après » dans le service de maternité de niveau IIb du Centre Hospitalier de Valence, sur l'ensemble des patientes présentant un DG et ayant accouché entre le 1er Janvier et le 31 Décembre 2008 pour l'échantillon 1 dit « avant », et entre le 1er Juillet 2009 et 30 Juin 2010 pour l'échantillon 2 dit « après ». Résultats : Il a été mis en évidence que le dépistage orienté en cas de facteurs de risque était davantage réalisé. Le délai de prise en charge était de fait moindre, et le nombre de consultation en endocrinologie supérieur, engendrant une diminution (bien que non significative) de toutes les complications de la grossesse étudiées. Introduction. Le risque de survenue d'un diabète de type 2 après un DG est multiplié par 6. Il impose un dépistage par une hyperglycémie provoquée orale (HGPO) ou une glycémie à jeun (GAJ) dans le post-partum. L'objectif principal de ce travail est de déterminer combien de femmes réalisent ce dépistage. Méthode. Etude quantitative épidémiologique descriptive rétrospective multicentrique. Inclusion de toutes les femmes ayant accouché entre le 01/01/2013 et le 31/10/2013 dans les hôpitaux de Grenoble (Centre Hospitalier Universitaire), Chambéry et Voiron ayant été codées « DG » à la sortie de la maternité. Recueil des données 4 mois après l'accouchement, par entretiens téléphoniques avec les médecins traitants, endocrinologues ou obstétriciens. Résultats. Le taux de réponse est de 92 % (428/466). 16,4 % des femmes [IC95 = 13-20] ont réalisé un test de dépistage du DT2 (56 GAJ et 14 HGPO). La prévalence du DT2 est de 4,3 % [-1,2- 9,8], celle de l'hyperglycémie non diabétique est de 2,9 % [-1,7- 7,5]. L'âge maternel n'est pas un facteur influençant la réalisation du dépistage (p=0,9497).
