Le but de ce travail est d'étudier l'apport du SCC dans l'évaluation du risque coronaire sur une population française de patients HF, et secondairement d'essayer de préciser les facteurs prédictifs d'un SCC élevé et donc d'un RCV accru. Il s'agit d'une étude rétrospective à propos de 220 patients asymptomatiques (54.8±7.7 ans, hommes/femmes=155/65) dont 104 avec HF (groupe HF+ défini par LDL-c ≥1.9g/l) et 116 sans HF (HF-), ayant bénéficié d'un SCC au CHU de Rennes, en prévention primaire du risque coronarien. Les facteurs de risque CV et le score de risque CV (score de Framingham évalué par l'équation d'Agostino) ont été analysés. Résultats : Les 2 populations HF+ et HF- présentent un risque CV équivalent sur les scores de risque CV. Elles ne diffèrent pas sur le SCC : (38.5 [0.5-139] vs 34 [0-153.5] p=0.74). L'appariement pour les facteurs de risque ne modifie pas ce résultat (58[0-217] vs 55[4-140] ; p=0.77). Il existe une corrélation modérée entre score de risque et SCC : r=0.42, p0.001. Le SCC permet de re-classifier 53 % de la population par rapport au risque coronaire. L'analyse multi-variée retrouve comme prédicateurs d'un SCC 100 l'âge chez l'homme et l'existence d'un traitement hypolipémiant. Conclusion : A risque cardiovasculaire équivalent, le SCC est similaire dans 2 populations avec et sans HF. Le SCC constitue un apport important dans l'évaluation du risque coronarien des patients HF.
