OBJECTIF : L'âge physiologique des sujets âgés est une nouvelle approche pas encore abordée en médecine générale et qui permettrait d'évaluer de manière subjective l'état de santé global des patients. Nous avons donc cherché à étudier les déterminants de l'âge physiologique des personnes âgées de plus de 65 ans, non institutionnalisées. MATÉRIEL ET MÉTHODE: S.AGES est une étude de cohorte multicentrique, française, où 760 médecins généralistes ont inclus 3434 patients de plus de 65 ans, non institutionnalisés, entre 2009 et 2011, atteints de diabète de type 2, d'ACFA ou de douleurs chroniques. L'évaluation de l'âge physiologique a été recueillie par les médecins traitants par questionnaires à l'inclusion, ainsi que les données socio-démographiques, les antécédents médicaux et comorbidités, les traitements, le mode de vie, l'autonomie, l'humeur, la fonction cognitive par les médecins traitants. Des modèles de régression logistique ont été utilisés pour analyser les variables influençant l'âge physiologique. RÉSULTATS: 3434 patients ont été inclus dont 1941femmes. 3427 avaient l'âge physiologique renseigné dont 22,7% des patients avaient un âge physiologique inférieur à leur âge chronologique, 67,2% égal et 10,1% supérieur. Ne pas être autonome augmentait le risque d'avoir un âge physiologique supérieur à son âge réel avec un OR = 2.43 [1.68 ; 3.52] entre 2 et 3 et OR = 5.04 [2.96 ; 8.61] entre 0 et 1. lnversement, ne pas être autonome diminuait le risque d'avoir un âge physiologique inférieur que son âge réel avec un OR = 0.52 [0.29-0.94] entre 2 et 3 et OR = 0.45 [0.32- 0.61] entre 0 et 1. Avoir une possible ou probable dépression augmentait significativement le fait de paraître plus vieux que son âge avec un OR=l.57 [1.37; 2.20]. Inversement, avoir une possible ou probable dépression diminuait significativement le fait de paraître plus jeune que son avec un OR=0.59 [0.44- 0.78]. Avoir un niveau scolaire supérieur augmentait le risque de paraître plus jeune que son âge avec un OR= 1.50 [1.12- 2.01]. Parmi les pathologies, les maladies artérielles et I'HTA diminuaient significativement le risque de paraître plus jeune que son âge contrairement à l'insuffisance cardiaque qui augmentait significativement le risque de paraître' plus vieux que son âge. La polymédication diminue le risque de paraître plus jeune que son âge avec un OR= 0.75 [0.58- 0.97]. CONCLUSION : Les médecins traitants semble avoir un bon sens clinique car ils évaluent instinctivement l'âge physiologique d'après les mêmes variables utilisées dans les scores gériatriques validés dont deux des facteurs essentiels sont l'autonomie et la dépression.

The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration.
