Le diabète est une maladie chronique, mondiale, en pleine expansion, avec une prise en charge actuelle non optimale. En effet les patients sont hospitalisés au moins une fois par an et 40% des personnes diabétiques ont un équilibre glycémique insuffisant pour éviter les complications. Ces vingt dernières années, une nouvelle prise en charge, via la télémédecine, a vu le jour. Plus récemment, l'élaboration du programme ETAPES (Expérimentations de Télémédecine pour l'Amélioration des Parcours En Santé) s'étendant à tout le territoire permet de prendre en charge les patients diabétiques grâce à la télésurveillance. Elle a été instaurée au CHU pour les patients mineurs et pour les adultes ainsi qu'en libéral par 3 équipes différentes. Ce projet de thèse est parti de la constatation que cette nouvelle prise en charge ne convenait pas à tous les patients, malgré leur inclusion selon les critères du programme. Nous avons souhaité avoir un pool de patients représentatif de l'ensemble des modes de prise en charge, c'est pourquoi nos collègues pédiatres et libéraux ont participé au projet. Nous avons inclus 55 patients et défini, parmi eux, un groupe de patients « adhérents » et un groupe « non adhérents » selon leur utilisation de l'outil après leur entrée dans le programme. Notre objectif principal est de pouvoir établir un profil type de patients à qui proposer la télésurveillance pour une meilleure adhésion, plus profitable. Nos critères secondaires sont étudiés au sein du seul groupe « adhérents » et comprennent l'équilibre glycémique à 3 mois du début de la prise en charge, l'évaluation globale de l'utilisation de l'outil et de la satisfaction de la télésurveillance, la détresse émotionnelle ainsi l'évaluation de l'autogestion du diabète. Notre étude met au jour une différence significative portant sur l'âge, avec une meilleure adhésion des patients âgés de plus de 50 ans (p = 0.04). Cette prise en charge a globalement amélioré l'équilibre glycémique de nos patients, notamment ceux inclus à la découverte de leur diabète. La plupart ont été satisfaits de la télésurveillance et nombre d'entre eux souhaitent la poursuivre. Il est également apparu que les patients diabétiques éprouvaient une forte détresse émotionnelle liée à leur pathologie.

CONTEXTE : La télésurveillance est en plein essor pour améliorer la prise en charge globale des patients diabétiques. Le programme d'expérimentation ETAPES a été mis en place afin d'évaluer les bénéfices sanitaires et les dépenses de santé du télésuivi dans les maladies chroniques. Notre étude a pour objectif d'évaluer l'impact à un an du télésuivi au sein du protocole d'expérimentation ETAPES au CHU Grenoble Alpes dans une population diabétique de type 1 et 2 traitée par insuline. MATERIELS ET METHODE : Etude rétrospective observationnelle. Inclusion des patients télésurveillés, via la plateforme MyDiabby, dans le cadre du programme ETAPES au sein du service de diabétologie du CHUGA de mai 2018 à mai 2019. Inclusion dans le programme ETAPES de minimum 6 mois, télésuivi sur une année maximum. RESULTATS : 65 patients ont été inclus. L'analyse du critère de jugement principal a pu être réalisée pour 54 patients. Au bout d'un an de suivi, la baisse de l'HbA1c est significative (p=0,0278) : à M0 l'HbA1c médiane était de 8,8% [8,2 ; 9,8] contre 8,2% [7,2 ; 9,2] à M12. On retrouve une baisse significative de l'HbA1c dès 6 mois de télésuivi (p=0 ,0055) avec une HbA1c médiane de 8,4% [7,5 ; 9]. CONCLUSION : La télésurveillance a permis aux patients diabétiques de retrouver un meilleur équilibre métabolique à 12 mois.
