Afin d'apprécier l'influence du strict contrôle métabolique obtenu par les méthodes de délivrance d'insuline optimisée par pompe portable (infusion continue sous-cutanée ou intrapéritonéale), nous avons fait l'étude rétrospective de 14 patients diabétiques en comparant leurs résultats après au minimum trois années d'utilisation de ce mode de traitement à ceux relevés pendant les douze mois qui ont précédé sa mise en route. Après avoir exposé les moyens employés pour évaluer les métabolismes glucidique et lipidique, les complications dégénératives liées ou non à la microangiopathie ainsi que les méthodes d'analyse statistique, nous avons présenté la population qui a été étudiée, les indications du traitement optimisé ainsi que les complications ou autres affections présentes à l'instauration de cette thérapeutique. Les résultats montrent une amélioration des métabolismes glucidique (sans normalisation) et lipidique, et une action modeste sur les complications dégénératives : légère aggravation de la rétinopathie, amélioration transitoire de la néphropathie et de la neuropathie. Ces résultats ne différent pas de ceux retrouvés dans la littérature pour des patients diabétiques aux caractéristiques équivalentes. Nous concluons que l'amélioration métabolique obtenue par les méthodes d'infusion continue d'insuline ne semble pas être en mesure d'enrayer l'évolution à long terme des complications dégénératives du diabète une fois installées. L'action préventive de tels modes de traitement sur l'apparition des complications est probable mais n'a pas été démontrée. Ces traitements devraient être proposés très précocement dans l'évolution du diabète.
