Face aux progrès rapides des traitements oncologiques (notamment par le passage des médicaments cytotoxiques conventionnels aux thérapies ciblées) même les pays les plus riches rencontrent des difficultés quant à la mise à disposition et au maintien de l'accès aux nouveaux médicaments. L'un des principaux défis est l'incertitude souvent importante entourant le degré de bénéfice clinique offert par un nouveau médicament au moment de son entrée sur le marché. Une comparaison internationale entre la France, l'Allemagne, le Royaume-Uni et le Canada a été réalisée dans le but d'étudier les déterminants de l'évaluation des médicaments en oncologie dans 4 pays en vue d'une future harmonisation européenne en trois étapes. Dans un premier temps, une recherche ad hoc a été conduite sur l'accès des traitements en oncologie dans les quatre pays à partir des documents publiés par les instances publiques afin d'étudier les critères définis comme étant pris en compte pour le remboursement/la prise en charge de ces traitements de manière spécifique à l'oncologie. Dans un second temps, une revue de la littérature a été effectuée afin de vérifier comment les différentes variables sont étudiées par les auteurs et si les éléments identifiés à partir des recommandations officielles sont réellement pris en compte dans les publications reportant l'analyse de ces différents systèmes. Enfin, une analyse des points d' amélioration identifiés a été réalisée dans le cadre de l'évaluation commune des technologies de santé. Bien que les recommandations réalisées par les agences d'évaluation des technologies de santé soient publiées, leur application en oncologie ne peut être capturée de manière totale en lien avec la transparence des évaluations rendues. Dans le cadre de l'évaluation commune des technologies de santé par la mise en place de l'action jointe 3 de l'EUnetHTA, les points de vigilance suivants ont été identifiés : la construction des essais cliniques et des études de données en vie réelle, l'accès conditionnel aux traitements innovants, la mise en place d'une échelle unique concernant le bénéfice clinique et la qualité de vie, la transparence des accords entre instances publiques et industriels, la transparence dans les évaluations et leur délai ainsi que la mise en place d'un dialogue précoce dans les évaluations et la prise en compte des données reportées par les patients.
