L'utérus cicatriciel est le facteur de risque principal de rupture utérine. Mais selon les recommandations professionnelles de 2012, la tentative de voie basse après césarienne est la solution à privilégier. Il est donc important de bien sélectionner les patientes à qui proposer une tentative de voie basse et d'identifier les facteurs de risques évitables de rupture utérine. Afin de réaliser une évaluation de nos pratiques, nous avons cherché à mettre en évidence les facteurs de risques, l'aspect clinique et les conséquences materno-fœtales de cette situation clinique. Il s'agit d'une étude de cohorte rétrospective observationnelle mono-centrique réalisée au sein de la maternité de type 3 du centre Aliénor d'Aquitaine, à Bordeaux, du 1 janvier 1995 au 31 décembre 2016. Toutes les patientes ayant eu un diagnostic de rupture utérine au cours d'une grossesse sont incluses dans notre étude. Le recueil de données est réalisé de façon rétrospective à partir des dossiers obstétricaux des patientes, informatiques et papiers. Il y a eu 92 194 accouchements entre le 01 janvier 1995 et le 31 décembre 2016 au CHU de Bordeaux. Notre population compte 74 ruptures utérines (8 pour 10 000 naissances). La rupture est survenue sur utérus sain dans 10 cas (1,2 pour 10 000 naissances) ; sur utérus cicatriciels dans 64 cas (74 pour 10 000 naissances). Parmi les 21 déclenchements sur utérus cicatriciels, une maturation cervicale par prostaglandines est réalisée chez 3 patientes (14,3%), et par ballons trans-cervicaux chez 6 patientes (28,6%). Dans la population globale, la durée moyenne du travail est de 8 heures, la dose totale d'ocytocique moyenne est de 12,9 UI et la dose maximale de 3,5UI/h. On retrouve un taux d'hystérectomie d'hémostase de 6,8% et un taux de décès néonataux de 3%. En cas d'IMG/MFIU la durée du travail est de 20 heures. En cas d'utérus sain on retrouve 30% d'hystérectomie d'hémostase et 16,7% de décès néonataux. Devant le risque très augmenté de rupture utérine en cas de maturation cervicale sur utérus cicatriciel, cette situation ne se retrouve à présent que rarement en salle de naissance. En revanche, une attention particulière doit être porté lorsque les doses utilisées d'ocytocique sont importantes et que le travail est long, surtout en cas d'utérus cicatriciel. Cependant, les situations d'utérus sains ou d'IMG/MFIU doivent aussi nous alerter. Car bien que ce soit des situations rares, les complications y sont majeures.
