Objectif : Réaliser un état des lieux de la présence de troubles sexuels des patients lors de leur hospitalisation, leurs prises en charge et d'évaluer la conscience des psychiatres de leur présence, leur importance. Méthode : Il s'agit d'une étude rétrospective observationnelle descriptive sur données existantes conduite au sein du Centre Hospitalier Guillaume Régnier au sein d'unités d'hospitalisation sur 3 mois. 577 dossiers ont été analysés. Pour les médecins, tous les psychiatres exerçant au sein du CHGR ainsi que tous les internes de Psychiatrie ont reçu via leur adresse mail professionnelle un lien vers un questionnaire anonyme en ligne. 56 réponses ont été récoltées. Résultats : Sur 577 dossiers analysés, on retrouve une trace écrite d'un questionnement du psychiatre par rapport à d'éventuels troubles sexuels sur 74 dossiers soit 12,8% des dossiers étudiés, dont 66 retrouvant un trouble sexuel soit 11,4%. Le sexe, l'âge et le statut marital représentaient des facteurs significatifs dans le dépistage de troubles sexuels. Sur les 66 troubles sexuels dépistés, 68,2% ont bénéficié d'une prise en charge soit 45 patients. Sur l'échantillon total, cela représente 7,8% des patients. Le questionnaire envoyé aux psychiatres du CHGR a permis de recueillir 56 réponses, dont la moitié sont des praticiens hospitaliers. Plus de 90% d'entre eux trouvait que la question de sexualité a de l'importance dans la prise en charge des patients. Discussion : Il s'agit de la première étude à investiguer la présence de troubles sexuels et leur prise en charge de patients hospitalisés dans des services psychiatriques permettant d'avoir une perspective de l'abord des dysfonctions sexuelles au sein d'unités d'hospitalisation. Concernant les limites, la prévalence de troubles sexuels et la diversité des prises en charge ne permettait pas de conclure de manière significative sur d'éventuels biais dans le traitement de troubles sexuels. De plus, les résultats sont dépendants de la tenue des dossiers et l'accessibilité des informations. Le questionnaire présentait un biais de déclaration. Conclusion : Cette étude a permis de mettre en évidence un manque de prise en charge de troubles sexuels avec un probable déficit de diagnostic médiée par l'âge, le genre et le statut relationnel des patients au sein de services universitaires de psychiatrie. Suite aux questionnaires distribués aux psychiatres exerçant au sein de l'établissement, nous avons pu observer une certaine disparité dans le recueil de données sexologiques, la retranscription de celles-ci selon différents critères. Les freins observés mettent en avant un manque de connaissances en matière de santé sexuelle ne permettant pas d'offrir l'information et le dépistage au patient. Suite à ce travail, il est proposé différentes stratégies de prise en charge des troubles sexuels ainsi qu'un document résumant l'intérêt et les différents moyens d'abord de la sexualité en psychiatrie.

How the insane asylum became a laboratory of democracy is revealed in this provocative look at the treatment of the mentally ill in nineteenth-century France. Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them.
