"INTRODUCCIÓN: La periodontitis es una enfermedad infecciosa causada por bacterias anaeróbicas gram negativas, que pueblan la placa subgingival. Las principales bacterias involucradas son: Porphyromonas gingivalis, Prevotella intermedi y Actinomices Actinomicetacomytans. Grossi et al., en 1997 sugirieron que la infección crónica de bacterias gram negativas y la endotoxemia crónica, como la que vemos en la enfermedad periodontal, podría inducir a resistencia a la insulina y empeorar el control metabólico en diabéticos. Distintas investigaciones han demostrado que el raspado y alisado radicular (RAR), mejoran el estado periodontal en pacientes con diabetes. Estos estudios también observaron el efecto de dicho tratamiento en los niveles de la hemoglobina glicosilada (HbA1c). Una revisión realizada por nuestro equipo en 2014 mostró que la mayoría de estudios encuentran esta relación, sin embargo hacían faltan estudios más homogéneos, con muestras más grandes y períodos de seguimiento más largos. HIPÓTESIS Y OBJETIVOS: Esto nos llevó a plantear la hipótesis de nuestro estudio "¿Hay una disminución en los niveles de Hemoglobina Glicosilada en pacientes con DM2 en respuesta al tratamiento periodontal no quirúrgico?". El objetivo principal fue determinar la asociación entre el tratamiento periodontal no quirúrgico y la variación de HbA1c en pacientes con DM2. Además de conocer la asociación dicho tratamiento y la mejora de los parámetros periodontales así como la respuesta microbiológica en pacientes con DM2. MATERIAL Y MÉTODOS: El estudio fue aprobado por el comité de ética del Hospital Odontológico de la Universidad de Barcelona en Marzo de 2013 (Anexo 1). Se siguieron las guías CONSORT para ensayos clínicos. Se realizó un estudio clínico controlado aleatorizado paralelo entre dos muestras de pacientes que presentan DM2 y periodontitis crónica generalizada. El tamaño muestral fue de 90 pacientes, y la duración del estudio de 6 meses. Los criterios de inclusión fueron los siguientes: pacientes con DM2, Periodontitis crónica generalizada según el criterio propuesto por Armitage 1999. Se excluyeron los pacientes con que hayan tomado antibiótico los últimos 15 días, y/o tratamiento periodontal en los últimos 6 meses. Todos los pacientes fueron visitados tres veces (al inicio, a los 3 y a los 6 meses), y todas las visitas fueron supervisadas por la doctoranda en la misma clínica. El grupo de tratamiento recibió tratamiento periodontal no quirúrgico mediante RAR completo en una única sesión, e instrucciones de higiene oral. El grupo control (grupo de tratamiento mínimo) recibió profilaxis dental al inicio del estudio, e instrucciones de higiene oral. Se anotó el valor de HbA1c y la glucemia al inicio y al final del estudio. En la visita inicial, a los 3 y a los 6 meses se registraron los parámetros periodontales y se tomaron muestras sulculares para hacer el estudio microbiológico.
