La lutte contre les cancers colorectaux est présentée en France par les pouvoirs publics comme une priorité. Ceci tient à leur fréquence (plus de 33 000 nouveaux cas par an) et à leur gravité (guérison d'un cancer sur deux seulement). Ce cancer a pour caractéristique d'être habituellement précédé pendant de nombreuses années par une tumeur bénigne, l'adénome. Les causes des cancers colorectaux ne sont encore que partiellement connues. L'excès calorique et la sédentarité sont des facteurs de risque bien établis alors que les légumes jouent un rôle protecteur. L'alcool et le tabac jouent un rôle important dans l'apparition et l'augmentation de taille des adénomes. Les polymorphismes des gènes codant pour des enzymes impliqués dans le métabolisme des aliments pourraient moduler le rôle des facteurs alimentaires dans la cancérogenèse. Le dépistage de masse correspond à une démarche validée qu'il est temps de mettre en place. Il concerne les individus âgés de 50 à 75 ans. Il est bien établi qu'il est possible de diminuer de 15 à 20% la mortalité par cancer colorectal en faisant un test Hemoccult tous les deux ans à condition que plus de la moitié de la population concernée participe régulièrement au dépistage et qu'une coloscopie soit faite en cas de test positif. Il existe des groupes à risque plus élevé. La coloscopie est recommandée chez les sujets ayant un apparenté atteint d'un cancer colorectal avant 60 ans ou deux apparentés atteints, dans la rectocolite hémorragique et la maladie de Crohn étendue (pancolite) évoluant depuis plus de 15 ans et chez les sujets ayant des antécédents personnels de cancer colorectal ou d'adénome de plus de 1 cm de diamtètre.
