Contexte : les structures hospitalières seules ne peuvent subvenir aux besoins en terme d'éducation thérapeutique du patient (ETP) pour l'insuffisance cardiaque (IC). La réalisation d'un diagnostic éducatif (DE) ambulatoire, en médecine générale permettrait de faciliter l'accessibilité de l'ETP aux patients et d'intégrer le médecin généraliste au programme éducatif. Objectif : nous avons évalué la faisabilité d'un DE pour l'IC en médecine générale. Méthodes : nous avons réalisé une étude descriptive auprès des médecins généralistes libéraux de la région Midi-Pyrénées ayant réalisé le DE pour l'IC, inspiré d'un programme validé et adapté à la médecine générale, chez un seul de leur patient insuffisant cardiaque, ambulatoire, non éduqué au préalable. Résultats : nous avons obtenu un taux de participation de 23% sur une cohorte de 100 participants. Le DE pour l'IC n'était pas réalisable en médecine générale pour 83% des médecins. Les principaux freins relatifs aux médecins étaient représentés par le manque de temps à 96%, de formation à 26%, de moyens matériels, humains et logistiques à 13% et l'absence de cotation et rémunération à 22%. Les principaux freins relatifs aux patients, selon les soignants étaient les niveaux cognitif et socio-culturel à 35% et le manque de motivation à 22%. Discussion : la réalisation du DE ambulatoire pour l'IC nécessite la prise en compte de freins à son application. Elle dépend, à ce jour, d'un travail en trinôme " patient-médecin généraliste-grandes instances de santé " dans un objectif de médecine de proximité, de santé publique et d'économie de la santé.

Objectifs : création d'un questionnaire qui a pour but de simplifier l'identification des objectifs éducatifs des patients insuffisants cardiaques chroniques à aborder à la consultation post hospitalisation avec le médecin généraliste. L'objectif secondaire de l'étude est de favoriser la communication entre les structures hospitalières et les cabinets de ville pour une meilleure prise en charge interdisciplinaire. Matériels et méthodes : étude transversale descriptive multicentrique avec un recrutement de janvier 2020 à août 2020. Le taux de participation est de 46 patients sur les 53 sollicités. Les critères d'inclusion étaient : patient atteint d'une insuffisance cardiaque chronique documentée à FEVG préservée ou réduite, un motif d'hospitalisation pour décompensation cardiaque aiguë, une décompensation cardiaque aiguë durant le séjour hospitalier, âgé de plus de 18 ans et capable de lire, d'écrire et de comprendre la langue française. Une analyse du questionnaire par tri à plat a été effectuée. Résultats : le questionnaire regroupe les 6 grands axes du diagnostic éducatif. La connaissance de la population permet une meilleure orientation dans le parcours de soin. Le suivi par un professionnel spécialisé était adéquat contrairement au suivi au domicile. Les connaissances théoriques sur la pathologie et les règles hygiéno-diététiques sont améliorables avec un manque d'application des recommandations. L'intérêt autour du ressenti du régime hyposodé donne une nouvelle approche à l'éducation thérapeutique. Conclusion : le bilan éducatif sous forme de questionnaire élaboré en fin d'hospitalisation facilite le travail du médecin généraliste et de ses coordinateurs. Une démarche éducative plus personnalisée et une meilleure compréhension des avantages et freins perçus pourrait tendre à une meilleure prise en charge globale du patients insuffisant cardiaque chronique. L'avènement des nouvelles technologies peut être une solution à un meilleur suivi à distance.

L'éducation thérapeutique est devenue un des fondements du traitement de toute maladie chronique, notamment de l'insuffisance cardiaque. En effet, il s'agit d'une pathologie fréquente et responsable de nombreuses hospitalisations pouvant être évitées pour certaines. L'éducation thérapeutique a pour but d'autonomiser le patient dans la gestion de sa maladie (traitement, signes de décompensation cardiaque, régime hyposodé, activité physique). Mais pour cela, sont requises certaines compétences propres à l'éducation thérapeutique : une formation adéquate du personnel, des outils de support, du temps à consacrer, et éventuellement un lieu dédié. Notre travail a consisté à suivre pendant 6 mois une cohorte de 40 patients : 20 patients éduqués à l'insuffisance cardiaque et 20 patients non éduqués. Après avoir comparé les connaissances des 2 groupes concernant leur maladie, nous avons étudié différents paramètres tels que, entre autres, le nombre de consultations chez le médecin traitant, chez le cardiologue, le nombre d'hospitalisations, les motifs de consultations et d'hospitalisations, les modifications de traitement. Notre étude a permis, outre de montrer l'intérêt évident de l'éducation thérapeutique, de définir les difficultés rencontrées dans ce projet à tous les niveaux (patients, professionnels de santé, financements, matériel, etc) dans la pratique quotidienne médicale de l'éducation thérapeutique.
